摘  要:目的 检测CD4^(+)细胞毒性相关辅助T淋巴细胞在人类正常睾丸及精原细胞瘤中的表达,探讨其在精原细胞瘤免疫微环境中的作用。方法 通过基因表达相互作用分析数据库(GEPIA)基因表达特征整合分析测序数据库分析CD4^(+)细胞毒性相关辅助T淋巴细胞标记分子CD4和颗粒酶K(GZMK)在正常睾丸及精原细胞瘤中的表达,并分析二者表达相关性及与肿瘤分期间的关系。采用免疫组化及双重荧光染色检测正常睾丸(n=8)与精原细胞瘤(n=12)组织中CD4、GZMK及CD4^(+)GZMK^(+)表达水平,并进行统计分析。结果 GEPIA基因表达谱分析结果显示精原细胞瘤CD4和GZMK表达水平高于正常睾丸,差异有统计学意义(P<0.05);高分期精原细胞瘤患者GZMK表达低于低分期精原细胞瘤,差异有统计学意义(P<0.05);CD4和GZMK表达水平呈正相关(r=0.71,P<0.05);免疫组化结果与GEPIA基因表达谱数据一致,即精原细胞瘤CD4和GZMK表达水平高于正常睾丸,差异有统计学意义(P<0.05);免疫荧光染色结果显示精原细胞瘤中CD4和GZMK表达共定位。结论 精原细胞瘤组织CD4、GZMK及CD4^(+)GZMK^(+)细胞表达水平较正常睾丸组织增高,提示CD4^(+)细胞毒性辅助性T淋巴细胞在精原细胞瘤免疫微环境中发挥重要作用。Objective To detect the expression of CD4^(+)cytotoxic-related helper T lymphocytes in normal testis and seminoma,and to explore their role in the immune microenvironment of seminoma.Methods The gene expression profiling interactive analysis(GEPIA) was used to analyze the expression of CD4^(+)cytotoxic-related helper T lymphocyte marker molecules CD4 and Granzyme K(GZMK) in normal testis and seminoma,and to analyze the correlation of their expression and the relationship between them and tumor stages.The expression levels of CD4,GZMK and CD4^(+)GZMK^(+)in normal testis(n=8) and seminoma(n=12) were detected by immunohistochemistry and double fluorescence staining,and statistical analysis was performed.Results The results of GEPIA gene expression profile analysis showed that the expression levels of CD4and GZMK in seminoma were higher than those in normal testis,and the differences were statistically significant(P<0.05).The expression of GZMK in high-stage seminoma was lower than that in low-stage seminoma,and the difference was statistically significant(P<0.05).CD4 and GZMK expression levels were positively correlated(r=0.71,P<0.05).The immunohistochemical results were consistent with GEPIA gene expression profile data,the expression levels of CD4 and GZMK in seminoma were higher than those in normal testis,and the differences were statistically significant(P<0.05).Immunofluorescence staining showed that CD4 and GZMK expression were co-localized in seminoma.Conclusion The expression levels of CD4,GZMK and CD4^(+)GZMK^(+)cells in seminoma tissues were higher than those in normal testicular tissues,suggesting that CD4^(+)cytotoxic T-helper lymphocytes play an important role in the immune microenvironment of seminoma.
