摘 要:目的 探讨结直肠管状腺瘤发生的危险因素。方法 选取2020年1月至2022年12月于临沂市人民医院诊治的155例患者作为研究对象,根据结肠镜下观察结果及组织病理学检查结果将其分为两组,其中77例结直肠管状腺瘤患者作为试验组,78例结肠镜下无息肉的患者作为对照组。比较两组一级亲属结直肠癌病史、吸烟史、饮酒史、年龄、性别、血压、体重指数、空腹血糖及血脂等,采用多因素logistic回归分析结直肠管状腺瘤发生的危险因素。结果 两组患者吸烟史、饮酒史、一级亲属结直肠癌病史、甘油三酯、高密度脂蛋白和空腹血糖比较,差异有统计学意义(P<0.05)。空腹血糖(β=0.747,OR=2.110,95%CI=1.389~3.206)、甘油三酯(β=1.513,OR=4.539,95%CI=2.119~9.719)、一级亲属结直肠癌病史(β=1.529,OR=4.615,95%CI=1.248~17.072)、饮酒史(β=1.120,OR=3.064,95%CI=1.273~7.377)和吸烟史(β=0.941,OR=2.564,95%CI=1.265~5.196)是结直肠管状腺瘤的危险因素(P<0.05),高密度脂蛋白(β=-1.095,OR=0.334,95%CI=0.117~0.954)是结直肠管状腺瘤的保护因素(P<0.05)。结论 结直肠管状腺瘤的发生受多种因素影响,空腹血糖升高、甘油三酯升高、一级亲属结直肠癌病史、饮酒史、吸烟史是其独立危险因素,高密度脂蛋白是其保护因素。Objective To explore the risk factors for colorectal tubular adenoma.Methods A total of 155 study subjects diagnosed and treated at Linyi People's Hospital from January 2020 to December 2022 were selected as research objects,they were divided into two groups based on the results of colonoscopy observation and histopathological examination.Among them,77 patients with colorectal tubular adenoma were selected as the experimental group,and 78 patients without polyps under colonoscopy were selected as the control group.History of colorectal cancer in first-degree relatives,smoking history,history of alcohol consumption,age,gender,blood pressure,body mass index,fasting plasma glucose and blood lipids of two groups were compared.The risk factors for the occurrence of colorectal tubular adenoma were analyzed by using multivariate logistic regression.Results The smoking history,alcohol consumption history,history of colorectal cancer in first-degree relatives,triglycerides,high-density lipoprotein and fasting plasma glucose between two groups were compared,the differences were statistically significant(P<0.05).Fasting plasma glucose(β=0.747,OR=2.110,95%CI=1.389-3.206),triglycerides(β=1.513,OR=4.539,95%CI=2.119-9.719),history of colorectal cancer in first-degree relatives(β=1.529,OR=4.615,95%CI=1.248-17.072),history of alcohol consumption(β=1.120,OR=3.064,95%CI=1.273-7.377) and smoking history(β=0.941,OR=2.564,95%CI=1.265-5.196) were risk factors for colorectal tubular adenoma(P<0.05),and high-density lipoprotein(β=-1.095,OR=0.334,95%CI=0.117-0.954) was a protective factor for colorectal tubular adenoma(P<0.05).Conclusion The occurrence of colorectal tubular adenoma is influenced by multiple factors,including elevated fasting plasma glucose,elevated triglycerides,history of colorectal cancer in first-degree relatives,history of alcohol consumption and smoking history,which are independent risk factors,and high-density lipoprotein is a protective factor.
