摘　　要：目的探讨神经纤维丝轻链蛋白(NfL)在炎症或免疫介导性脑小血管病(CSVD)中的水平及对疾病的影响。方法回顾性分析2019年9月至2021年9月九江市中医医院收治的60例脑血管病患者的临床资料,根据有无CSVD分为对照组(无CSVD)与观察组(有CS�VD),每组30例。所有患者均接受MRI检查,并于临床治疗前检测其血清NfL水平。观察患者影像学表现,比较两组血清NfL水平,并分析血清NfL水平对CSVD的预测价值。结果MRI检查影像学结果显示,观察组中皮质下小梗死灶3例,腔隙性梗死4例,脑白质高信号10例,血管周围间隙4例,脑微出血8例,脑萎缩1例;观察组血清NfL水平为(21.63±2.53)ng/L,高于对照组的(16.26±2.04)ng/L,差异有统计学意义(P<0.05)。ROC结果显示,血清NfL水平评估CSVD的AUC为0.896,当NfL取19.345 ng/L时,可获得最佳评估效能。结论炎症或免疫介导性CSVD患者具有明显的影像学信号特征,且血清NfL在炎症或免疫介导性CSVD患者中呈过表达,检测血清NfL对评估CSVD具有一定的价值。Objective To explore the level of neurofilament light(NfL)on inflammatory or immune-mediated cerebral small vessel diseases(CSVD)and its impact on the disease.Methods The clinical data of 60 patients with cerebrovascular disease admitted to Jiujiang Hospital of Traditional Chinese Medicine from September 2019 to September 2021 were retrospectively analyzed,they were divided into the control group(without CSVD)and observation group(with CSVD)according to the presence or absence of CSVD,with 30 cases in each group.All patients underwent MRI,the serum NfL level was detected before clinical treatment.The imaging findings of patients were observed,the serum NfL level of the two groups was compared,and the predictive value of serum NfL level on CSVD was analyzed.Results MRI showed that in the observation group,there were 3 cases of subcortical small infarcts,4 cases of lacunar infarcts,10 cases of high signal intensity of cerebral white matter,4 cases of perivascular space,8 cases of cerebral microbleeds and 1 case of cerebral atrophy.The serum NfL level in the observation group was(21.63±2.53)ng/L,which was higher than(16.26±2.04)ng/L in the control group,and the difference was statistically significant(P<0.05).The results of ROC showed,according to the results of assessment on the level of serum NfL CSVD AUC 0.896,when the NfL take 19.345 ng/L,can obtain optimal evaluation effectiveness.Conclusion Patients with inflammatory or immune-mediated CSVD have obvious imaging signal characteristics,and serum NfL is overexpressed in patients with inflammatory or immune-mediated CSVD,and the detection of serum NfL has certain value in evaluating CSVD.
