低剂量CT联合七种自身抗体检测对早期肺癌的诊断价值

摘　　要：目的 探讨低剂量CT(Low-dose computed tomography, LDCT)联合七种自身抗体检测对早期肺癌的诊断价值。方法 选取2022年1月至2023年6月间就诊于牡丹江医学院附属第二医院的肺结节患者51例。通过手术后病理确诊为肺癌的患者32例,非肺癌患者19例,分别将其设为肺癌组与对照组。所有患者术前均进行LDCT扫描并采用酶联免疫吸附法检测七种自身抗体(P53、PGP9.5、SOX2、GAGE7、GBU4-5、MAGEA1、CAGE)水平。采用受试者工作特征曲线(ROC)分析LDCT、七种自身抗体及二者联合对早期肺癌的诊断价值。结果 51例患者中,LDCT诊断阳性35例、阴性16例,5例肺癌误诊为良性病变,8例肺部良性病变误诊为肺癌。七种自身抗体P53、PGP9.5、SOX2、GAGE7、GBU4-5、MAGEA1、CAGE的曲线下面积(AUC)分别为0.567、0.552、0.646、0.583、0.682、0.697、0.635,均大于0.500。其中,GBU4-5、MAGEA1的P值<0.05,差异有统计学意义。LDCT、七种自身抗体及二者联合诊断早期肺癌的AUC分别为0.711、0.723、0.859。LDCT诊断早期肺癌的灵敏度、特异度分别为84.4%、57.9%,七种自身抗体检测的灵敏度、特异度分别为65.6%、78.9%,二者联合灵敏度、特异度可分别提高到87.5%、84.2%。结论 LDCT和七种自身抗体检测对早期肺癌均有较高的诊断价值,二者联合检查可进一步提高诊断效能。Objective Exploring the diagnostic value of low-dose CT(LDCT)combined with seven autoantibodies for early lung cancer.Methods Select fifty-one patients with pulmonary nodules who visited the Second Affiliated Hospital of Mudanjiang Medical University from January 2022 to June 2023.thirty-two patients were diagnosed with lung cancer through postoperative pathological examination and nineteen non-lung cancer patients were divided into lung cancer group and control group,respectively.All patients underwent LDCT scans before surgery and were tested for seven types of autoantibodies using enzyme-linked immunosorbent assay.The diagnostic value of LDCT,seven autoantibodies,and their combination in early lung cancer was analyzed using the receiver operating characteristic curve(ROC).Results Among the fifty-one patients,LDCT diagnosis was positive in thirty-five cases and negative in sixteen cases.Five cases of lung cancer were misdiagnosed as benign lesions,and eight cases of benign lung lesions were misdiagnosed as lung cancer.The area under the curve(AUC)of the seven autoantibodies P53,PGP9.5,SOX2,GAGE7,GBU4-5,MAGEA1,and CAGE for lung cancer were 0.567,0.552,0.646,0.583,0.682,0.697,and 0.635,respectively,all greater than 0.5.Among them,the P-values of GBU4-5 and MAGEA1 are less than 0.05,which has statistical significance.The AUC values of LDCT,seven autoantibodies,and their combination in the diagnosis of early lung cancer were 0.711,0.723,and 0.859,respectively.The sensitivity and specificity of LDCT in diagnosing early lung cancer were 84.4%and 57.9%,respectively.The sensitivity and specificity of seven types of autoantibodies were 65.6%and 78.9%,respectively.The combined sensitivity and specificity of the two can be increased to 87.5%and 84.2%,respectively.Conclusion LDCT and seven types of autoantibodies have high diagnostic value for early lung cancer,and combined examination can further improve diagnostic efficiency.
