基于交互式模式的运动锻炼对强直性脊柱炎患者的效果观察

摘　　要：目的:探究基于交互式模式的运动锻炼应用于强直性脊柱炎患者的效果,即观察其对脊柱功能状态、炎症指标、辅助性T细胞17(T helper cells-17,Th17)以及调节性T细胞(T regulatory cell,Treg)水平的影响。方法:选取我科2020年7月至2022年2月期间106例强直性脊柱炎患者,根据抽签法分为对照组和观察组。其中对照组53例给予综合护理及运动干预,观察组53例给予基于交互式模式的运动锻炼,干预3 m后对比两组患者脊柱功能状态:采用强直性脊柱炎病情活动指数(Bath ankylosing spondylitis disease activity index,BASDAI)、强直性脊柱炎功能指数(Bath ankylosing spondylitis functional index,BASFI)评估脊柱功能;采用酶联免疫吸附法检测炎症指标如转化生长因子β-(Transforming growth factor-β,TGF-β)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α),乳胶免疫比浊法检测C反应蛋白(C-reactive protein,CRP),用魏氏法检测红细胞沉降率(Erythrocyte sedimentation rate,ESR)水平;采用全自动流式细胞仪检测免疫指标如Th17、Treg水平。结果:干预前两组各项指标均无统计学意义(P>0.05);干预后观察组Treg水平高于对照组,CRP、TNF-α、TGF-β、ESR、Th17水平、BASDAI、BASFI评分均低于对照组,差异有统计学意义(P<0.05)。结论:基于交互式的护理模式下运动锻炼对强直性脊柱炎患者干预,可有效降低机体炎症反应,提高免疫能力,促进脊柱功能恢复。Objective:To explore the effect of interactive exercise on patients with ankylosing spondylitis,that is,to observe the effects on spinal functional status,inflammatory markers,levels of T helper cells-17(Th17)and T regulatory cell(Treg).Methods:A total of 106 patients with ankylosing spondylitis in our department from July 2020 to February 2022 were selected and divided into control group and observation group according to lottery method.Among them 53 patients in the control group were given comprehensive nursing and exercise intervention,and 53 patients in the observation group were given exercise based on interactive mode.After 3 months of intervention,the spinal function status of patients in the two groups was compared:Bath ankylosing spondylitis disease activity index(BASDAI),Bath ankylosing spondylitis functional index(BASFI)to evaluate spinal function;Transforming growth factor-β(TGF-β)and Tumor necrosis factor-α(TNF-α)were used to detect inflammatory indicators.The emulsion immunoturbidimetry was used to detect C-reactive protein(CRP)and the Erythrocyte sedimentation rate(ESR)level was detected by Weil's method.Automatic flow cytometry was used to detect the level of immune indicators such as Th17 and Treg.Results:Before intervention,there was no statistical significance in two groups(P>0.05).After intervention,Treg level of observation group was higher than control group,CRP,TNF-α,TGF-β,ESR,Th17 level,BASDAI and BASFI score were lower than control group,the difference was statistically significant(P<0.05).Conclusion:Exercise based on interactive nursing mode in patients with ankylosing spondylitis intervention,can effectively reduce the body inflammatory response,improve the immune ability,promote the recovery of spinal function.
