血清miR-182-5p、miR-372-3p表达水平与急性脑梗死患者病情及预后的关系

摘　　要：目的 检测急性脑梗死患者的血清miR-182-5p、miR-372-3p表达水平,并探讨其与患者病情及预后的关系。方法 选取2020年3月至2022年10月安阳市人民医院神经内科收治的136例急性脑梗死患者纳入脑梗死组,另选取本院同期136例体检健康志愿者作为对照组。根据病情程度将脑梗死组患者分为轻度组(n=42)、中度组(n=59)和重度组(n=35)。根据患者90 d随访情况分为预后良好组103例和预后不良组33例。采用qRT-PCR法检测两组受检者的血清miR-182-5p、miR-372-3p表达水平,并比较各组受试者miR-182-5p、miR-372-3p表达水平和美国国立卫生研究院脑卒中量表(NIHSS)评分。采用Spearman相关性分析急性脑梗死患者血清miR-182-5p、miR-372-3p与NIHSS评分的相关性;采用多因素Logistic回归分析急性脑梗死预后的影响因素;绘制受试者工作特征曲线(ROC)分析miR-182-5p、miR-372-3p预测急性脑梗死患者预后的价值。结果 脑梗死组患者的血清miR-182-5p、miR-372-3p表达水平分别为0.56±0.16、0.39±0.11,明显低于对照组的0.99±0.23、1.00±0.24,差异均有统计学意义(P<0.05);随着脑梗死患者病情程度的加重,轻度组、中度组、重度组患者的血清miR-182-5p、miR-372-3p表达水平逐渐降低,而NIHSS评分逐渐增加,差异均有统计学意义(P<0.05);经Spearman相关性分析结果显示,急性脑梗死患者血清miR-182-5p、miR-372-3p水平与NIHSS评分均呈负相关(r=-0.483、-0.648,P<0.05);预后良好组患者的血清miR-182-5p、miR-372-3p表达水平分别为0.62±0.17、0.42±0.12,明显高于预后不良组的0.39±0.12、0.30±0.09,差异均有统计学意义(P<0.05);经多因素Logistic回归分析结果显示,miR-182-5p和miR-372-3p低表达以及重度梗死是急性脑梗死预后不良的影响因素(P<0.05);经ROC分析结果显示,血清miR-182-5p、miR-372-3p水平联合预测急性脑梗死预后不良的曲线下面积(AUC)明显大于miR-182-5p单独预测的AUC (Z=1.991,P=0.046)Objective To detect the expression levels of miR-182-5p and miR-372-3p in serum of pa-tients with acute cerebral infarction,and to explore their relationship with the patients'disease condition and prog-nosis.Methods A total of 136 patients with acute cerebral infarction admitted to the Department of Neurology,Any-ang People's Hospital from March 2020 to October 2022 were selected as the cerebral infarction group,and 136 healthy volunteers for medical checkups were collected as the control group.Patients with cerebral infarction were grouped into mild group(n=42),moderate group(n=59),and severe group(n=35)according to the severity of the disease.According to the 90 d follow-up of patients,they were grouped into 103 cases in good prognosis group and 33 cases in poor progno-sis group.The serum expression levels of miR-182-5p and miR-372-3p were detected by qRT-PCR,and the expression levels of miR-182-5p and miR-372-3p and the scores of National Institutes of Health Stroke Scale(NIHSS)were com-pared among the groups of subjects.Spearman correlation was used to analyze the correlation between serum miR-182-5p,miR-372-3p and NIHSS score in patients with acute cerebral infarction.Multivariate Logistic regression was applied to analyze the influence factors of prognosis in acute cerebral infarction.ROC curve was drawn to analyze the prognostic value of miR-182-5p and miR-372-3p in patients with acute cerebral infarction.Results The expres-sion levels of miR-182-5p and miR-372-3p in the serum of patients with cerebral infarction were 0.56±0.16 and 0.39±0.11,respectively,significantly lower than 0.99±0.23 and 1.00±0.24 of the control group(P<0.05).As the severity of ce-rebral infarction worsened,the expression levels of serum miR-182-5p and miR-372-3p in the mild,moderate,and se-vere groups gradually decreased,while the NIHSS score gradually increased,with statistically significant differences(P<0.05).Spearman correlation analysis showed that the serum levels of miR-182-5p and miR-372-3p in patients with acute cerebra
