摘　　要：目的 探讨原发性肝癌术后种植转移的危险因素及预后。方法 回顾性收集2015年1月至2021年12月收治于徐州市中心医院的原发性肝癌术后腹腔种植转移(腹腔种植转移组28例)及无腹腔种植转移(无腹腔种植转移组294例)患者的临床和随访资料,分析腹腔种植转移与临床特征及总生存期的关系。结果 腹腔种植转移组和无腹腔种植转移组在肝包膜侵犯、肿瘤破裂和肿瘤分化程度方面比较,差异均具有统计学意义(均P<0.05)。腹腔种植转移组和无腹腔种植转移组术后3年总生存率分别为14.7%和59.3%,平均总生存期为11.8和43.4个月;中位总生存期比较,差异具有统计学意义(6.5个月vs 52个月,P<0.01)。结论 肝癌术后种植转移与包膜侵犯、肿瘤破裂及肿瘤低分化密切相关,严重影响患者总生存期。Objective To investigate the risk factors and prognosis of intraperitoneal seeding after liver resection in primary liver cancer patients.Methods The clinical and follow-up data of primary liver cancer patients with intraperitoneal seeding(the intraperitoneal seed-ing group,n=28)and without intraperitoneal seeding(the intraperitoneal non-seeding group,n=294)were retrospectively collected from January 2015 to December 2021 in Xuzhou Central Hospital.The relationship between intraperitoneal seeding and clinical characteristics and the correlation between intraperitoneal seeding and overall survival were analyzed.Results There were statistically significant differ-ences in liver capsule involvement,tumor rupture and tumor differentiation between the intraperitoneal seeding and non-seeding groups(all P<0.05).The 3-year overall survival rates of the intraperitoneal seeding and non-seeding groups were 14.7%and 59.3%,with the average overall survival time of 11.8 months and 43.4 months.The median overall survival time of the intraperitoneal seeding and non-seeding groups was 6.5 months and 52 months(P<0.01).Conclusions Intraperitoneal seeding after liver cancer surgery is related to capsule in-volvement,tumor rupture and poor differentiation and seriously affects the overall survival of liver cancer patients.
