摘 要:目的探讨血清淀粉样蛋白A(SAA)、胞角蛋白19片段(CYFRA)21-1和癌胚抗原(CEA)联合检测诊断非小细胞肺癌(NSCLC)的临床意义。方法回顾性选取2020年5月至2021年5月抚州市第一人民医院收治的30例NSCLC患者作为肺癌组,另选取同期抚州市第一人民医院收治的30例肺部良性疾病患者作为对照组。所有患者均进行血清SAA、CYFRA21-1、CEA水平检测,比较两组血清SAA、CYFRA21-1、CEA水平,比较单一指标检测及联合检测的灵敏度、特异度及准确度。结果肺癌组血清CEA及CYFRA21-1水平均高于对照组,差异有统计学意义(P<0.05);两组血清SAA水平比较差异无统计学意义。联合检测灵敏度、准确度均明显高于血清SAA、CEA、CYFRA21-1单一检测,差异有统计学意义(P<0.05),血清SAA、CEA、CYFRA21-1单一检测灵敏度、准确度两两比较差异均无统计学意义;不同检测方法特异度比较差异无统计学意义。结论血清SAA、CYFRA21-1、CEA联合检测在NSCLC诊断中具有较高的灵敏度、特异度和准确度,能为临床控制病情及判断患者预后提供可靠的参考依据,具有较高的临床应用价值。Objective To explore the clinical significance of combined detection of serum amyloid A(SAA),cytokeratin 19 fragment(CYFRA)21-1 and carcinoembryonic antigen(CEA)in the diagnosis of non-small cell lung cancer(NSCLC).Methods 30 patients with NSCLC admitted to Fuzhou First People's Hospital from May 2020 to May 2021 were retrospectively selected as the lung cancer group,and another 30 patients with be-nign lung diseases admitted to Fuzhou First People's Hospital during the same period as the control group.Serum levels of SAA,CYFRA21-1 and CEA were detected in all patients,the serum levels of SAA,CYFRA21-1,and CEA were compared between the two groups,and the sensitivity,spec-ificity,and accuracy of single indicator detection and combined detection were compared.Results The serum CEA and CYFRA21-1 levels in the lung cancer group were higher than those in the control group,and the differences were statistically significant(P<0.05);there was no statistically significant difference in serum SAA level between the two groups.The sensitivity and accuracy of combined detection were significantly lower than those of single detection of serum SAA,CEA,and CYFRA21-1,and the differences were statistically significant(P<0.05);there was no significant difference in sensitivity and accuracy of single detection of SAA,CEA and CYFRA21-1;there was no significant difference in specificity of different detection methods.Conclusion The combined detection of serum SAA,CYFRA21-1 and CEA has high diagnostic sensitivity,specificity and accu-racy in the diagnosis of non-small cell lung cancer,can provide a reliable reference basis for clinical control of the disease and judging the prognosis of patients,and which has high clinical application value.
