小切口白内障手法碎核摘除术治疗硬核白内障的临床效果及对术后恢复的影响

摘　　要：目的:探讨小切口白内障手法碎核摘除术治疗硬核白内障的临床效果及对术后恢复的影响。方法:选取2022年1月—2023年2月丹阳市中医院收治的90例硬核白内障患者。按照患者的治疗方案分为对照组和观察组,各45例。对照组行囊外白内障摘除术治疗,观察组行小切口白内障手法碎核摘除术治疗。比较两组术后视力水平、角膜散光水平、眼压水平、角膜内皮细胞密度及并发症发生率。结果:术前,两组视力水平比较,差异无统计学意义(P>0.05);术后1个月、3个月,观察组视力水平高于对照组,差异有统计学意义(P<0.05)。术前,两组角膜散光水平比较,差异无统计学意义(P>0.05);术后1个月、3个月,观察组角膜散光水平低于对照组,差异有统计学意义(P<0.05)。术前、术后3个月,两组眼压水平比较,差异无统计学意义(P>0.05);术后1个月,观察组眼压水平低于对照组,差异有统计学意义(P<0.05)。术前、术后1个月,两组角膜内皮细胞密度比较,差异无统计学意义(P>0.05);术后3个月,观察组角膜内皮细胞密度高于对照组,差异有统计学意义(P<0.05)。观察组并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论:小切口白内障手法碎核摘除术治疗硬核白内障取得满意治疗效果,患者术后视力、眼压、散光均取得更理想的恢复效果,并且并发症少,安全性更高。Objective:To investigate the clinical effect and postoperative recovery of hardcore cataract treated by small incision cataract manual nuclear fragmentation extraction.Method:A total of 90 patients with hardcore cataract treated in Danyang Hospital of Traditional Chinese Medicine from January 2022 to February 2023 were selected.According to the treatment plan,the patients were divided into control group and observation group,with 45 cases in each group.The control group was treated with extracapsular cataract extraction,and the observation group was treated with small incision cataract manual nuclear fragmentation extraction.The visual acuity level,corneal astigmatism level,intraocular pressure level,density of corneal endothelial cells and complication rate of the two groups were compared.Result:Before surgery,there was no significant difference in visual acuity level between the two groups(P>0.05).At 1 month and 3 months after surgery,the visual acuity level of observation group were higher than those of control group,and the differences were statistically significant(P<0.05).Before surgery,there was no significant difference in corneal astigmatism level between the two groups(P>0.05).At 1 month and 3 months after surgery,the level of corneal astigmatism in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Before surgery and 3 months after surgery,there were no significant differences in intraocular pressure between the two groups(P>0.05).After 1 month after surgery,the intraocular pressure level of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).There were no significant differences in the density of corneal endothelial cells between the two groups before and 1 month after surgery(P>0.05).At 3 months after surgery,the density of corneal endothelial cells in observation group was higher than that in control group,and the difference was statistically significant(P<0.05
