摘　　要：目的:研究血液灌流(HP)联合连续性静脉-静脉血液滤过(CVVH)治疗高脂血症性急性胰腺炎的效果。方法:选取2021年10月—2023年2月毕节市中医医院收治的85例高脂血症性急性胰腺炎患者作为观察对象。根据治疗的先后顺序进行数字编号,号数从1号到85号,将号数打乱排序后随机将其分为对照组(42例)和观察组(43例)。两组均给予常规治疗,对照组给予CVVH治疗,观察组给予HP联合CVVH治疗。比较两组临床疗效,治疗前后相关指标、病情严重程度及炎症程度。结果:观察组总有效率为95.35%,高于对照组的80.95%,差异有统计学意义(P<0.05)。治疗后,两组甘油三酯(TG)、淀粉酶(AMS)、C反应蛋白(CRP)水平降低,观察组TG、AMS、CRP水平均低于对照组,差异有统计学意义(P<0.05)。治疗后,两组急性生理与慢性健康状况评分Ⅱ(APACHEⅡ评分)、SIRS评分均低于治疗前,观察组APACHEⅡ评分、全身炎症反应综合征(SIRS)评分均低于对照组,差异有统计学意义(P<0.05)。结论:高脂血症性急性胰腺炎患者应用HP联合CVVH治疗,能明显降低血脂,减轻炎症反应,效果显著。Objective:To investigate the effect of hemoperfusion(HP)combined with continuous veno-venous hemofiltration(CVVH)in the treatment of hyperlipidemic acute pancreatitis.Method:Eighty-five patients with hyperlipidemic acute admitted to Bijie Hospital of Traditional Chinese Medicine from October 2021 to February 2023 were selected as the observation objects.According to the order of treatment,the numbers are numbered from 1 to 85.After shuffling the numbers,they are randomly divided into control group(42 cases)and observation group(43 cases).Both groups were given conventional treatment,the control group was given CVVH treatment,and the observation group was given HP combined CVVH treatment.The clinical efficacy,relevant indicators,the severity of the disease and the degree of inflammation before and after treatment were compared between the two groups.Result:The total effective rate of the observation group was 95.35%,which was higher than 80.95%of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of triglyceride(TG),amylase(AMS)and C-reactive protein(CRP)in the two groups decreased,and the levels of TG,AMS and CRP in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the scores of acute physiology and chronic health status scoring systemⅡ(APACHEⅡ)and systemic inflammatory response syndrome(SIRS)in both groups were lower than those before treatment,and the scores of APACHEⅡand SIRS in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:The application of HP combined with CVVH treatment in patients with hyperlipidemic acute pancreatitis can significantly reduce blood lipids,alleviate inflammatory reactions,and have a significant effect.
