摘　　要：目的 探讨miRNA-138-5p、miRNA-34b在肺癌中的表达及与患者临床特征和预后的关系。方法 选取151例肺癌患者,均进行手术切除治疗,取术后肺癌组织及癌旁组织,采用实时荧光定量逆转录聚合酶链反应(RT-PCR)检测miRNA-138-5p、miRNA-34b水平。比较不同临床特征、不同预后肺癌患者肺癌组织中miRNA-138-5p、miRNA-34b水平,肺癌患者预后的影响因素采用多因素Logistic回归分析。绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC),评估miRNA-138-5p、miRNA-34b单独及联合检测对肺癌患者预后的预测价值。结果 肺癌组织中miRNA-138-5p、miRNA-34b水平均明显低于癌旁组织,差异均有统计学意义(P﹤0.01)。肿瘤直径≥2 cm、TNM分期为Ⅲ期、有淋巴结转移、分化程度为低分化的肺癌患者肺癌组织中miRNA-138-5p、miRNA-34b水平分别明显低于肿瘤直径﹤2 cm、TNM分期为Ⅰ~Ⅱ期、无淋巴结转移、分化程度为中高分化的患者,差异均有统计学意义(P﹤0.01)。至随访结束,151例肺癌患者中,预后良好119例,预后不良32例。预后不良肺癌患者肺癌组织中miRNA-138-5p、miRNA-34b水平均明显低于预后良好患者,差异均有统计学意义(P﹤0.01)。多因素Logistic回归分析结果显示,肿瘤直径≥2 cm、TNM分期为Ⅲ期、有淋巴结转移、分化程度为低分化、miRNA-138-5p水平降低、miRNA-34b水平降低均是肺癌患者预后的独立危险因素(P﹤0.05)。ROC曲线显示,miRNA-138-5p、miRNA-34b联合检测预测肺癌患者预后的AUC为0.951(95%CI:0.918~0.984),高于二者单独检测的0.603、0.572。结论 miRNA-138-5p、miRNA-34b在肺癌组织中异常低表达,且与患者临床特征、预后密切相关,有望成为评估肺癌恶性程度及预后的重要分子标志物。Objective To investigate the expression of miRNA-138-5p and miRNA-34b in lung cancer and their relationship with clinical characteristic and prognosis of patients.Method A total of 151 lung cancer patients were selected and all underwent surgical resection.Postoperative lung cancer tissues and adjacent tissues were collected,and the levels of miRNA-138-5p and miRNA-34b were detected by real-time fluorescence quantitative reverse transcription-polymerase chain reaction(RT-PCR).The levels of miRNA-138-5p and miRNA-34b in lung cancer tissues of lung cancer patients with different clinical characteristics and prognosis were compared.The influencing factors for the prognosis of lung cancer patients were analyzed by multivariate Logistic regression.The receiver operating characteristic(ROC) curve was drawn,and the area under the curve(AUC) was calculated to evaluate the predictive value of miRNA-138-5p and miRNA-34b alone and combined detection for the prognosis of lung cancer patients.Result The levels of miRNA-138-5p and miRNA-34b in lung cancer tissues were significantly lower than those in adjacent tissues,and the differences were statistically significant(P<0.01).The levels of miRNA-138-5p and miRNA-34b in lung cancer tissues of patients with tumor diameter ≥2 cm,TNM stage III,lymph node metastasis,and poorly differentiated were significantly lower than those with tumor diameter <2 cm,TNM stage I-II,no lymph node metastasis,and moderate to high differentiation,and the differences were statistically significant(P<0.01).By the end of the follow-up,among the 151 lung cancer patients,119 had a good prognosis and 32 had a poor prognosis.The levels of miRNA-138-5p and miRNA-34b in lung cancer tissues of lung cancer patients with poor prognosis were significantly lower than those of patients with good prognosis,and the differences were statistically significant(P<0.01).The results of multivariate Logistic regression analysis showed that tumor diameter ≥2 cm,TNM stage III,lymph node metastasis,poorly differentiated,do
