原发性胆囊癌的CT征象与临床特点分析

摘　　要：目的探讨原发性胆囊癌的CT征象与临床特点。方法选取2020年5月—2023年6月本院收治的42例原发性胆囊癌患者进行回顾性分析,将其分为恶性组,选取同期来本院治疗的40例良性胆囊肿瘤患者作为良性组,所有患者CT诊断资料完整,对比42例原发性胆囊癌患者CT诊断与病理诊断对原发性胆囊癌不同分型的检出率。建立受试者工作特征(ROC)曲线分析CT对于原发性胆囊癌的诊断效能。结果CT诊断42例原发性胆囊癌患者发现有1例患者被误诊为良性胆囊肿瘤,2例患者漏诊,CT诊断检出率与病理诊断对比无明显差异(P>0.05);CT诊断原发性胆囊癌的特异度灵敏度与病理诊断对比无明显差异(P>0.05)。结论CT对原发性胆囊癌的诊断准确性较高,并且通过CT图像重建可明确原发性胆囊癌的不同类型。Objective To investigate the CT signs and clinical features of primary gallbladder carcinoma.Methods 42 patients with primary gallbladder cancer admitted to our hospital from May 2020 to June 2023 were retrospectively analyzed,and they were divided into malignant group.The CT diagnostic data of all patients were complete.The detection rates of different types of primary gallbladder cancer were compared between CT diagnosis and pathological diagnosis in 42 cases.Receiver operating characteristic(ROC)curve was established to analyze the diagnostic efficiency of CT in primary gallbladder cancer.Results Among 42 patients with primary gallbladder cancer diagnosed by CT,1 patient was misdiagnosed as benign gallbladder tumor,and 2 patients were missed.There was no significant difference between the detection rate of CT diagnosis and the pathological diagnosis(P>0.05).There was no significant difference between the specificity sensitivity of CT diagnosis and the pathological diagnosis(P>0.05).Conclusion CT is accurate in the diagnosis of primary gallbladder cancer,and the different types of primary gallbladder cancer can be identified by CT image reconstruction.
