摘 要:目的利用国产PET-MF-2V-IT-I模块自动化合成^(18)F-PSMA-DCFPyL,并分析其在前列腺癌双示踪剂PET/CT显像中的应用效果。方法利用国产PET-MF-2V-IT-I模块,将^(18)F与前体PSMA在50℃下通过亲核反应自动化合成^(18)F-PSMA-DCFPyL,并对产品进行质量分析。对2例前列腺癌患者行^(18)F-PSMA-DCFPyL和^(18)F-FDG双示踪剂PET/CT显像并比较显像结果。结果^(18)F-PSMA-DCFPyL总合成时间为65 min,合成产率为(19.8±0.8)%(n=22),放化纯度>95%,稳定性好,产品符合无菌和无热原要求。与^(18)F-FDG相比,^(18)F-PSMA-DCFPyL对分化较好的前列腺癌原发灶及转移灶的显像更清晰,而^(18)F-FDG对分化差的前列腺癌病灶或合并小细胞癌的显像更清晰。结论利用国产PET-MF-2V-IT-I模块自动化合成^(18)F-PSMA-DCFPyL,方法简单、稳定,合成产品能满足临床需求。^(18)F-PSMADCFPyL在前列腺癌患者体内的显像效果良好,与^(18)F-FDG PET/CT显像相比优势明显,同时也是对其的有效补充。Objective To automate the synthesis of ^(18)F-PSMA-DCFPyL by using domestically produced PET-MF-2V-IT-I module,and to analyze its application in dual tracer PET/CT imaging of prostate cancer.Methods Using the domestically produced PET-MF-2V-IT-I module,^(18)F was automatically synthesized with precursor PSMA via nucleophilic reaction at 50℃,and the quality of the product was analyzed.^(18)F-PSSMA-DCFPyL and ^(18)F-FDG dual tracer PET/CT imaging was performed in 2 patients with prostate cancer and the imaging results were compared.Results The total synthesis time of ^(18)F-PSMA-DCFPyL was 65 minutes,the synthesis yield is(19.8±0.8)%(n=22).The purification purity was>95%,with the good stability,the sterility and non pyrogen of the product can meet the requirements.Compared with ^(18)F-FDG,^(18)F-PSMA-DCFPyL has clearer imaging of well differentiated prostate cancer primary and metastatic lesions,while ^(18)F-FDG has clearer imaging of poorly differentiated prostate cancer lesions or combined small cell carcinoma.Conclusion With the use of domestically produced PET-MF-2V-IT-I modules for automated synthesis of ^(18)F-PSMA-DCFPyL,the method is simple and stable,and the synthetic product can meet clinical needs.^(18)F-PSMA-DCFPyL has a good imaging effect in prostate cancer patients,and has obvious advantages over ^(18)F-FDG PET/CT imaging,and is also an effective supplement to it.
