超声引导下经皮穿刺注射平阳霉素硬化治疗肝血管瘤的有效性及安全性观察

摘　　要：目的观察超声引导下经皮穿刺注射平阳霉素硬化治疗肝血管瘤的有效性及安全性。方法收集2015年1月至2022年12月于首都医科大学附属北京友谊医院接受超声引导下平阳霉素注射硬化治疗的114例肝血管瘤患者的临床资料进行回顾性分析。观察技术上的成功率,术后可能的并发症、血常规、肝功能及凝血功能指标;术后3、6和12个月进行随访,通过视觉模拟量表(VAS)评分评估患者的症状缓解情况,同时观察血管瘤最大切面的最大径线、体积变化。结果114例患者完成随访,技术上的成功率是100%。所有患者无严重并发症发生,血常规、肝功能及凝血功能指标术前术后均在正常范围。患者术后3、6和12个月症状严重程度VAS评分、肝血管瘤最大径线、肝血管瘤体积均低于/小于术前[(3.6±1.8)、(2.6±2.0)、(2.3±1.9)分比(7.6±1.2)分,(5.9±2.3)、(5.1±1.8)、(4.3±1.7)cm比(7.9±3.0)cm,44.9(20.9,102.4)、29.1(11.4,62.8)、18.0(6.3,41.9)cm^(3)比116.5(61.4,264.8)cm^(3)],差异均有统计学意义(均P<0.001)。结论超声引导下经皮穿刺注射平阳霉素硬化治疗肝血管瘤是一种安全有效的方法。Objective To observe the effectiveness and safety of ultrasound-guided percutaneous injection of pingyangmycin in the sclerotherapy of hepatic hemangioma.Methods A retrospective analysis was conducted on the clinical data of 114 patients who underwent ultrasound-guided pingyangmycin injection sclerotherapy for liver hemangioma at Beijing Friendship Hospital,Capital Medical University from January 2015 to December 2022.The technical success rate,possible postoperative complications,blood routine,liver function,and coagulation function indicators were observed;follow up was conducted at 3,6,and 12 months after surgery,and the patient′s symptom relief was evaluated using the Visual Analogue Scale(VAS) score.At the same time,the maximum diameter and volume changes of the largest section of the hemangioma were observed.Results Totally 114 patients completed follow-up,with a technical success rate of 100%.No serious complications occurred in all patients,and blood routine,liver function,and coagulation function indicators were within normal ranges before and after surgery.At 3,6,and 12 months after surgery,the patient′s symptom severity VAS score,maximum diameter of hepatic hemangioma,and volume of hepatic hemangioma were all lower/smaller than those before surgery[(3.6±1.8),(2.6±2.0),(2.3±1.9) vs(7.6±1.2);(5.9±2.3),(5.1±1.8),(4.3±1.7)cm vs(7.9±3.0)cm;44.9(20.9,102.4),29.1(11.4,62.8),18.0(6.3,41.9)cm^(3) vs 116.5(61.4,264.8)cm^(3)](all P<0.001).Conclusion Ultrasound-guided percutaneous injection of pingyangmycin for sclerotherapy of hepatic hemangioma is a safe and effective method.
