摘　　要：目的对比不同引流管在非小细胞肺癌患者电视胸腔镜肺叶切除术中的临床使用效果。方法采用前瞻性随机对照研究方法,纳入行电视胸腔镜肺叶切除术治疗的116例非小细胞肺癌患者为研究对象,采用抽签法将其分为对照组(58例)与观察组(58例),对照组采用常规28F胸腔闭氏引流管,观察组采用16F超细引流管,对比两组手术临床指标、术后引流量、术后疼痛程度、引流时间、住院时间及并发症。结果两组手术时间、术中淋巴结清扫数目、术中出血量、术后1 d、2 d、3 d时引流量、引流时间比较,差异无统计学意义(P>0.05);观察组住院时间短于对照组,术后1 d、2 d、3 d时视觉模拟评分法(VAS)评分、并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论与常规28F胸腔闭式引流管比较,16F超细引流管可减轻非小细胞肺癌患者电视胸腔镜肺叶切除术患者术后疼痛,减少并发症发生。Objective To compare the clinical effects of video-assisted thoracoscopic lobectomy for non-small cell lung cancer patients with different drainage tubes.Methods A prospective randomized controlled study was conducted in 116 patients with non-small cell lung cancer who were scheduled to undergo video-assisted thoracoscopic lobectomy.They were divided into the control group(58 cases)and the observation group(58 cases)by drawing lots.The control group used the conventional 28F closed thoracic drainage tube,and the observation group used the 16F ultra-fine drainage tube.The clinical indexes,postoperative drainage volume,postoperative pain,drainage time,hospital stay and complications of the 2 groups were compared.Results There was no statistical significant difference between the 2 groups in operation time,number of lymph node dissection,amount of bleeding during operation,drainage volume and drainage time at 1,2 and 3 days after operation(P>0.05).The hospital stay in the observation group was shorter than that in the control group,and the VAS score at 1,2,and 3 days after operation and the incidence of complications were lower than that in the control group,with a statistically significant difference(P<0.05).Conclusion Compared with the conventional 28F closed thoracic drainage tube,16F ultra-fine drainage tube can reduce the postoperative pain and complications in patients with non-small cell lung cancer undergoing video-assisted thoracoscopic lobectomy.
