结肠癌组织中PADI3、CDK5RAP3及PDCD4的表达及其与临床病理特征和预后关系的研究

摘　　要：目的分析结肠癌组织中肽基精氨酸脱亚胺酶3(PADI3)、周期素依赖性激酶5激活结合蛋白3(CDK5RAP3)及程序性细胞死亡因子4(PDCD4)的表达及其与临床病理特征和预后关系。方法回顾性分析,选取2016年3月1日至2019年12月28日在徐州医科大学附属淮安医院接受治疗的70例结肠癌患者为研究对象,检测PADI3、CDK5RAP3、PDCD4的表达量,观察其与结肠癌临床病理特征的关系,并采用双变量Spearman相关性分析PADI3、CDK5RAP3、PDCD4与临床病理特征的相关性;同时建立多因素Logistic分析模型,分析PADI3、CDK5RAP3、PDCD4对预后的影响因素。结果结肠癌PADI3蛋白低表达、高表达率分别为38.57%、61.43%,CDK5RAP3蛋白低表达、高表达率分别为58.57%、41.43%,PDCD4蛋白低表达、高表达率分别为60.00%、40.00%。低表达PADI3、CDK5RAP3和PDCD4的染色强度×细胞阳性百分比乘积值水平均明显低于高表达,差异均有统计学意义(P<0.05)。低表达PADI3的mRNA水平均明显低于高表达,低表达CDK5RAP3和PDCD4 mRNA水平均明显高于高表达,差异均有统计学意义(P<0.05)。PADI3高表达TNMⅢ~Ⅳ期、有淋巴结转移率明显高于低表达,CDK5RAP3、PDCD4低表达TNMⅢ~Ⅳ期、有淋巴结转移率均明显高于高表达,差异均有统计学意义(P<0.05)。结肠癌组织PADI3与TNM分期、淋巴结转移呈正相关(P<0.05);CDK5RAP3、PDCD4与TNM分期、淋巴结转移呈负相关(P<0.05)。中位无疾病进展(PFS)、总生存期(OS)比较,PADI3高表达明显比低表达短;CDK5RAP3、PDCD4低表达明显比高表达短,差异均有统计学意义(P<0.05)。多因素Logistic分析结果显示,TNM分期、淋巴结转移、PADI3、CDK5RAP3、PDCD4均是影响结肠癌PFS和OS的独立危险因素(P<0.05)。结论PADI3、CDK5RAP3、PDCD4与结肠癌分期、淋巴结转移及其预后密切相关,其表达变化可评估结肠癌的疾病严重程度及其预后。Objective To analyze the expression of peptidylarginine deiminase 3(PADI3),cyclin-dependent kinase 5 activated binding protein 3(CDK5RAP3)and programmed cell death factor 4(PDCD4)in colon cancer tissues and their relationship with clinicopathological features and prognosis.Methods For retrospective analysis,70 colon cancer patients who received treatment at the Affilicated Huai'an Hospital of Xuzhou Medical University from March 1,2016 to December 28,2019 were selected for the study,and the expressions of PADI3,CDK5RAP3,and PDCD4 were detected to observe their relationship with clinicopathological characteristics and prognosis of colon cancer,and bivariate Spearman correlation analysis of PADI3,CDK5RAP3,PDCD4 with clinicopathological features;meanwhile,a multifactor logistic model was established to analyze the influencing factors of PADI3,CDK5RAP3,PDCD4 on prognosis.Results In colon cancer,low and high expression of PADI 3 was 38.57%and 61.43%,CDK5RAP3 low and high expression were 58.57%and 41.43%,and PDCD4 was 60.00%and 40.00%,respectively.The levels of the product of percentage positive cells by staining intensity of low expression PADI3,CDK5RAP3,PDCD4 were significantly lower than those of high expression,and the differences were statistically significant(P<0.05).The level of mRNA of low expression PADI3 was lower than high expression,and the levels of mRNA of low expression CDK5RAP3 and PDCD4 were higher than high expression,and the differences were statistically significant(P<0.05).High expression PADI3 in TNMⅢ/Ⅳstage and lymph node metastasis rate were significantly higher than low expression PADI3,the low expression of CDK5RAP3 and PDCD4 in TNMⅢ/Ⅳstage and the lymph node metastasis rate were significantly higher than those with high expression and the differences were statistically significant(P<0.05).PADI3 was positively correlated with TNM stage and lymph node metastasis(P<0.05),CDK5RAP3 and PDCD4 were negatively correlated with TNM stage and lymph node metastasis(P<0.05).Compared with the media
