摘 要:目的探讨急性脑梗死(ACI)患者外周血正五聚体蛋白3(PTX3)、骨桥蛋白水平与神经功能、梗死面积及预后的关系。方法回顾性选取2020年5月至2022年6月于河北北方学院附属第一医院接受诊治的92例ACI患者作为研究对象,设为观察组;选取同期46名健康体检者,设为对照组。对比两组研究对象的外周血PTX3、骨桥蛋白水平;分析不同临床病理特征[性别、年龄、高血压史、糖尿病史、吸烟史、饮酒史、美国国立卫生研究院卒中量表(NIHSS)评分、梗死最大直径、改良Rankin量表(mRs)评分]ACI患者的PTX3、骨桥蛋白水平变化;采用分层回归模型分析PTX3、骨桥蛋白水平与ACI患者神经功能、梗死面积及预后的关系。结果观察组患者的PTX3、骨桥蛋白水平分别为(14.17±3.55)pg/mL、(7.69±2.08)mg/L,均明显高于对照组[(7.82±1.52)pg/mL、(5.02±1.05)mg/L],差异均有统计学意义(P<0.05)。不同性别、年龄、高血压史、糖尿病史、吸烟史、饮酒史患者的PTX3、骨桥蛋白水平比较,差异均无统计学意义(P>0.05);不同NIHSS评分、梗死最大直径、mRs评分患者的PTX3、骨桥蛋白水平比较,差异均有统计学意义(P<0.05)。分层回归模型显示,梗死最大直径会对PTX3产生显著的正向影响关系(P<0.05),而NIHSS评分、mRs评分并不会对PTX3产生影响关系(P>0.05)。NIHSS评分、mRs评分会对骨桥蛋白产生显著正向影响关系(P<0.05),而梗死最大直径不会对骨桥蛋白产生影响关系(P>0.05)。结论ACI患者外周血PTX3、骨桥蛋白表达随着神经功能缺损严重程度、脑梗死面积的增加及预后差呈显著上升趋势。Objective To investigate the relationship between the levels of pentamer protein(PTX3)and osteopontin in peripheral blood of patients with acute cerebral infarction(ACI)and neurological function,infarct size and prognosis.Methods Ninety-two patients with ACI who were treated in the First Affiliated Hospital of Hebei North University from May 2020 to June 2022 were retrospectively selected as the study objects and set as the observation group.A total of 46 healthy subjects at the same period were selected as control group.The levels of PTX3 and osteopontin in peripheral blood were compared between the two groups.The changes of PTX3 and osteopontin levels in ACI patients with different clinicopathological characteristics[gender,age,history of hypertension,history of diabetes,history of smoking,history of drinking,National Institute of Health stroke scale(NIHSS)score,maximum diameter of infarction,modified Rankin scale(mRs)score]were analyzed.The relationship between PTX3,osteopontin levels and neurological function,infarct size,and prognosis in ACI patients was analyzed using a stratified regression model.Results The levels of PTX3 and osteopontin in observation group were(14.17±3.55)pg/mL and(7.69±2.08)mg/L,respectively,which were significantly higher than those in the control group[(7.82±1.52)pg/mL and(5.02±1.05)mg/L],the differences were statistically significant(P<0.05).There was no statistically significant difference in PTX3 and osteopontin levels among patients with different sex,age,history of hypertension,diabetes,smoking and drinking(P>0.05).The PTX3 and osteopontin levels of patients with different NIHSS scores,maximum infarct diameter and mRs scores were compared,and the differences were statistically significant(P<0.05).The hierarchical regression model showed that the maximum infarct diameter had a significant positive effect on PTX3(P<0.05),while NIHSS score and mRs score had no effect on PTX3(P>0.05).NIHSS score and mRs score had a significant positive effect on osteopontin(P<0.05),but the maxim
