摘 要:目的探究微量血与常量血血清样本在0~6岁儿童维生素D水平检测中的应用效果。方法前瞻性选取2020年2月至2022年2月银川市妇幼保健院儿保科门诊就诊的147名0~6岁健康体检儿童作为本次研究的对象,同时对入组对象采集微量血(末梢取血)和常量血(静脉取血),测定其25-羟基维生素D3[25(OH)D3]水平,并对末梢采样结果进行校正。比较两种采血方式下25(OH)D3的水平,并比较不同采血季节(春季、夏季、秋季、冬季)时25(OH)D3的水平差异。记录不同采血方式下,平均穿刺时间、次数,呼吸频率上升值及疼痛情况,比较两种采血方式的采血满意度。结果静脉取血和末梢取血测定的25(OH)D3水平中位数分别为29.10(24.40,34.30)ng/mol和28.80(22.20,36.30)ng/mol,两种采血方式测定的25(OH)D3水平差异无统计学意义(P>0.05)。不同采血季节下静脉取血和末梢取血时25(OH)D3的检测结果组间比较差异有统计学意义(P<0.05),以夏季最高[静脉取血29.78(22.46,36.48)ng/mol,末梢取血28.46(21.59,36.66)ng/mol]和春季最低[静脉取血26.11(23.15,32.14)ng/mol,末梢取血25.41(22.56,33.46)ng/mol],组间比较差异有统计学意义(P<0.05)。静脉取血和末梢取血平均穿刺时间及视觉模拟评分法(VAS)评分比较,差异均无统计学意义(P>0.05),末梢取血的呼吸频率上升值为(8.13±0.98%),高于静脉取血[(7.16±0.67%)],末梢取血平均穿刺次数为(1.45±0.49)次,少于静脉取血[(2.09±0.57)次],差异均有统计学意义(P<0.05)。末梢取血的取样满意度为95.24%,高于静脉采血(87.07%),差异有统计学意义(P<0.05)。结论对0~6岁儿童采取微量血进行维生素D检测,结果与常量血检测无明显差异,但可减少穿刺次数,提高满意度,且以夏季检测值最高,春季检测值最低。Objective To explore the comparison of trace blood and constant serum samples in the detection of vitamin D levels in children aged 0-6 years.Methods A prospective study was conducted,and 147 healthy children aged 0-6 years who visited the outpatient of Department of Pediatric Care,Yinchuan Maternal and Child Health Hospital from February 2020 to February 2022 were selected as the subjects.Microsamples(peripheral blood sampling)and conventional blood samples(venous blood sampling)were collected from the enrolled subjects,and their levels of 25-hydroxyvitamin D3[25(OH)D3]were measured,with correction applied to the peripheral sampling results.The levels of 25(OH)D3 were compared between the two blood collection methods,and differences in 25(OH)D3 levels during different blood collection seasons(spring,summer,autumn,and winter)were also compared.The average puncture time,number of punctures,increase in respiratory rate,and pain level were recorded for both blood collection methods,and the satisfaction with the two blood collection methods was compared.Results The medion levels of 25(OH)D3 measured through venous and peripheral blood sampling were 29.10(24.40,34.30)ng/mol and 28.80(22.20,36.30)ng/mol,respectively,there was no statistically significant difference between the two methods(P>0.05).There was statistically significant difference in the detection results of 25(OH)D3 between venous and peripheral blood sampling taken in different blood collection seasons between groups(P<0.05),with the highest levels in summer[venous:29.78(22.46,36.48)ng/mol,peripheral:28.46(21.59,36.66)ng/mol]and the lowest in spring[venous:26.11(23.15,32.14)ng/mol,peripheral:25.41(22.56,33.46)ng/mol].There were no statistically significant differences in the average puncture time and visual analogue scale(VAS)scores between venous and peripheral blood collection(P>0.05),however,the increase in respiratory rate was higher of peripheral blood sampling was(8.13±0.98%),which was higher than that of venous blood sampling[(7.16±0.67%)],and th
