摘　　要：系统性红斑狼疮(SLE)是一种自身免疫性疾病,全身多系统均可受累,其中肺部是常见的受累系统之一。弥漫性泛细支气管炎(DPB)是一种双肺弥漫性的呼吸性细支气管区域的慢性炎症性疾病,若不及时诊治,最终可能会发展成呼吸衰竭甚至导致病死。目前国内尚无SLE合并DPB的病例报道,加之此类病例主要以咳嗽咳痰及活动后喘累、呼吸困难等不典型临床表现为主,难以鉴别,容易造成误诊、漏诊。本文报告1例SLE合并DPB的治疗过程并回顾相关文献,分析此类病例的发病机制、鉴别要点,以期为临床诊治此类患者提供参考思路。Systemic lupus erythematosus(SLE)is an autoimmune disease that affects multiple systems throughout the body,with the lungs being one of the commonly affected parts.Diffuse panbronchiolitis(DPB)is a chronic inflammatory disease characterized by diffuse respiratory bronchiolitis in both lungs.If left untreated,it may eventually develop into respiratory failure and even lead to death.At present,there are no reported cases of SLE combined with DPB in China.In addition,such cases are mainly characterized by atypical clinical manifestations such as cough,phlegm,wheezing and dyspnea after activity,etc.,making it difficult to distinguish,which can easily lead to misdiagnosis and missed diagnosis.This article reports the treatment process of a case of SLE combined with DPB and reviews relevant literature,analyzing the pathogenesis and identification points of this case,in order to provide reference ideas for clinical dia gnosis and treatment of this type of patient.
