HPV、TCT及阴道镜对宫颈癌筛查的临床效果研究

摘　　要：目的探讨在宫颈癌筛查中人乳头瘤病毒(HPV)、液基薄层细胞学检测(TCT)及阴道镜检查的应用价值。方法选取农村35~64岁两癌普查女性464例,均予以TCT、HPV检测以及阴道镜检查,以组织学检测结果作为金标准,分析三种检测方式的应用价值。结果464例受试者经组织病理学检查确诊为宫颈病变者148例。TCT诊断准确率为77.16%,与金标准一致性差(Kappa=0.40);HPV诊断准确率为89.87%,与金标准一致性较好(Kappa=0.77);阴道镜诊断准确率为91.59%,与金标准一致性较好(Kappa=0.81)。TCT联合HPV诊断低级别病变阳性检出率高于炎症(P<0.05);高级别病变阳性检出率高于低级别病变(P<0.05)。TCT、HPV、阴道镜单独检测对宫颈病变的检出率分别为40.54%、89.19%、90.54%,联合检测检出率为100%。结论在宫颈癌筛查中TCT、HPV检测与阴道镜检查均有一定应用价值,联合检测能提高对宫颈病变的检出率。Objective To explore the application value of human papillomavirus(HPV),TCT and colposcopy in cervical cancer screening.Methods 464 women aged 35~64 years in rural areas were selected to undergo TCT,HPV and colposcopy.Histological results were used as the gold standard to analyze the application value of the three detection methods.Results Of the 464 subjects,148 had cervical lesions confirmed by histopathological examination.The diagnostic accuracy of TCT was 77.16%,which was not consistent with gold standard(Kappa=0.40).The accuracy of HPV diagnosis was 89.87%,which was in good agreement with the gold standard(Kappa=0.77).The diagnostic accuracy of colposcopy was 91.59%,which was in good agreement with the gold standard(Kappa=0.81).The positive rate of TCT combined with HPV was higher than that of inflammation(P<0.05).The positive detection rate of high-grade lesions was higher than that of low-grade lesions(P<0.05).The detection rates of TCT,HPV and colposcopy alone were 40.54%,89.19%and 90.54%,respectively,and the detection rates of combined detection were 100%.Conclusion TCT,HPV detection and colposcopy have certain application value in cervical cancer screening,and the combined detection can improve the detection rate of cervical lesions.
