摘 要:目的探究血清垂体瘤转化基因1(pituitary tumor tansforming gene1,PTTG1)、血管内皮生长因子(vascular endothelial growth factor,VEGF)、胸苷激酶1(thymidine kinase 1,TK1)水平与喉癌、下咽癌短期预后的相关性及预后不良影响因素。方法选取2017年1月至2021年12月青岛大学附属青岛市中心医院就诊的117例喉癌及下咽癌患者作为观察组,另选取同期就诊的142例喉良性病变患者作为对照组。酶联免疫吸附法检测两组患者的血清PTTG1、VEGF、TK1水平。对观察组患者进行为期1年的术后短期预后随访,根据随访结果将其分为预后良好组和预后不良组。比较两组患者的临床资料及术前血清PTTG1、VEGF、TK1水平,Spearman相关性分析血清PTTG1、VEGF、TK1水平与患者短期预后不良的相关性,单因素和多因素Logistic回归分析患者预后不良的影响因素。结果观察组血清PTTG1(139.18±25.13)pg/ml、VEGF(466.19±27.14)pg/ml、TK1(4.09±0.59)pmol/L均显著高于对照组,差异有统计学意义(P<0.001)。观察组117例患者中,96例预后良好,21例预后不良。预后良好组与预后不良组患者的性别、年龄、肿瘤直径、组织学分化程度、临床分期、淋巴结转移、血清PTTG1、VEGF、TK1比较,差异有统计学意义(P<0.05)。Spearman相关性分析显示,血清PTTG1、VEGF、TK1水平与短期预后不良呈正相关(r=0.593、0.544、0.698,P<0.01)。Logistic分析显示,年龄、肿瘤直径、组织学分化程度、临床分期、淋巴结转移、血清PTTG1、血清VEGF、血清TK1均为预后不良的独立影响因素(P<0.05)。结论血清PTTG1、VEGF、TK1水平与喉癌、下咽癌短期预后存在相关性,患者短期预后不良影响因素较多,可综合应用于疾病的预后评估中。Objective To explore the correlation between serum PTTG1,VEGF,TK1 levels and the short-term prognosis of laryngeal and hypopharyngeal cancer and the adverse prognostic factors.Methods A total of 117 patients with laryngeal and hypopharyngeal cancer admitted to the affiliated Qingdao central hospital of Qingdao University from January 2017 to December 2021 were selected as the observation group.Another 142 patients with benign laryngeal lesions who were diagnosed during the same period were selected as the control group.Enzyme linked immunosorbent assay was used to detect the serum levels of PTTG1,VEGF,and TK1 in two groups of patients.Patients in the observation group were followed up for a short-term postoperative prognosis of one year,and were divided into a good prognosis group and a poor prognosis group based on the follow-up results.Clinical data and preoperative serum PTTG1,VEGF,TK1 levels were compared between two groups of patients.Spearman correlation analysis was used to investigate the correlation between serum PTTG1,VEGF,TK1 levels and short-term poor prognosis in patients.Univariate and multivariate logistic regression analysis was used to investigate the influencing factors of poor prognosis in patients.Results The serum PTTG1(139.18±25.13)pg/ml,VEGF(466.19±27.14)pg/ml,and TK1(4.09±0.59)pmol/L in the observation group were significantly higher than those in the control group,with statistically significant differences(P<0.001).Among the 117 patients in the observation group,96 had a good prognosis and 21 had a poor prognosis.There were statistically significant differences in gender,age,tumor diameter,histological differentiation,clinical staging,lymph node metastasis,serum PTTG1,VEGF,and TK1 between the patients with good prognosis and those with poor prognosis(P<0.05).Spearman correlation analysis showed that serum levels of PTTG1,VEGF,and TK1 were positively correlated with poor short-term prognosis(r=0.593,0.544,0.698,P<0.01).Logistic analysis showed that age,tumor diameter,histological differ
