摘　　要：目的:探讨透明冠修复儿童上颌乳前牙龋损的效果及对继发龋发生率的影响。方法:选取2020年12月-2021年12月笔者医院收治的88例上颌乳前牙龋损患儿,按照修复方法不同将其分为对照组(44例,108颗患牙)和研究组(44例,112颗患牙)。对照组行传统前牙复合树脂直接充填修复治疗,研究组行透明冠联合复合树脂修复治疗。比较两组修复前及修复1年后牙周指标、修复效果、不良事件及患儿的家属满意度。结果:修复后1年,两组牙菌斑指数(Plaque index,PLI)、牙龈出血指数(Bleeding index,BI)及龈沟液肿瘤坏死因子-α(Tumor necrosis factorα,TNF-α)、白介素-8(Interleukin 8,IL-8)水平均低于修复前,且研究组PLI、BI及龈沟液TNF-α、IL-8水平均低于对照组(P<0.05);研究组修复成功率高于对照组(P<0.05),研究组继发龋发生率、边缘染色发生率及充填体脱落发生率均低于对照组(P<0.05),研究组家属满意度评分高于对照组(P<0.05)。结论:透明冠修复可有效改善上颌乳前牙龋损患儿牙周状况,修复效果良好,可降低继发龋发生率,减少并发症的发生,患儿家属更满意。Objective To investigate the effect of transparent crown restoration on the caries of children's maxillary deciduous anterior teeth and its influence on the incidence of secondary caries.Methods From December 2020 to December 2021,88 children with caries of maxillary primary anterior teeth were selected and divided into control group(44 cases,108 teeth)and study group(44 cases,112 teeth)according to the repair methods.The control group was treated with direct filling of traditional anterior teeth with composite resin,while the study group was treated with transparent crown combined with composite resin.The periodontal indicators,restorative effects,adverse events,and family satisfaction were compared between the two groups before and 1 year after restoration.Results One year after restoration,the plaque index(PLI),gingival bleeding index(BI)and gingival sulcus fluid tumor necrosis factorα(TNF-α)and interleukin 8(IL-8)levels in both groups were lower than before restoration.The levels of PLI,BI and gingival sulcus fluid TNF-αand IL-8 in the study group were lower than in the control group(P<0.05).The repair success rate of the study group was higher than that of the control group(P<0.05).One year after restoration,the incidence of secondary caries,the incidence of marginal staining and the incidence of filling loss in the study group were lower than in the control group(P<0.05).The score of family satisfaction in the study group was higher than that in the control group(P<0.05).Conclusion Transparent crown restoration can effectively improve the periodontal condition of children with caries of maxillary deciduous primary anterior teeth,with good restorative effects.In addition,transparent crown restoration can also reduce the incidence of secondary caries and reduce the incidence of complications.And the family members of the children are more satisfied.
