新活素对心脑肾保护作用的研究进展

摘　　要：利钠肽作为目前临床最常用的反映心脏负荷敏感标志物之一,在心力衰竭方面具有特别意义,尤其是心房利钠肽(ANP)和脑利钠肽(BNP),两者都是主要由心脏分泌的激素,尤其是BNP和前体NT-proBNP水平在心力衰竭患者中升高,对于鉴别心力衰竭有重要作用。冻干重组人脑利钠肽(新活素)常用于急性心力衰竭治疗,通过拮抗血管紧张素和肾上腺素系统,起到扩张动静脉及冠状动脉,降低心脏前后负荷。除此以外,新活素还表现出改善心室重构,延缓细胞纤维化,限制心肌梗死后梗死范围以及调节炎症反应等作用。在肾脏方面,多项临床研究表明新活素预防应用可起到肾脏保护作用,如冠脉介入术后造影剂肾损伤等。在神经系统方面,新活素也表现出保护作用。本文针对新活素对心脑肾保护作用的研究进展进行综述,以期为新活素在临床上更好应用提供帮助。As one of the most commonly used sensitive biomarkers reflecting cardic load in clinical practice,natriuretic peptides(NPs)have special significance in heart failure field,especially atrial natriuretic peptide(ANP)and brain natriuretic peptide(BNP),both of which are hormones mainly secreted by the heart,the levels of which,especially BNP and the precursor NT-proBNP are elevated in patients with heart failure.So they play an important role in the identification of heart failure.Lyophilized recombinant human brain natriuretic peptide Xinhuosu is commonly used in the treatment of acute heart failure,by antagonizing the angiotensin and adrenaline systems,it dilates artery and vein,and coronary arteries,reduces pre-and post cardiac load.In addition,Xinhuosu also shows the effects of improving ventricular remodeling,delaying cellular fibrosis,limiting the infarct size after myocardial infarction,and regulating inflammatory response.In the kidney system,a number of clinical studies have shown that the preventive use of Xinhuosu can play a protective role in the kidneys,such as contrast agent kidney injury afer percutaneous coronary intervention.In the nervous system field,Xinhuosu also shows protective effects.This article provides a review of the research progress on the protective effects of Xinhuoxin on the heart,brain and kidney,with the aim of providing assistance for its better clinical application.
