氩氦刀冷冻消融术治疗局限性前列腺癌的疗效分析

摘　　要：目的探讨氩氦刀冷冻消融术治疗局限性前列腺癌的疗效及安全性。方法回顾性分析2014年1月至2019年12月本院行直肠超声引导下经会阴前列腺氩氦刀冷冻消融的21例局限性前列腺癌患者的临床资料并进行随访研究。评估患者的血清总前列腺特异性抗原(tPSA)、国际前列腺症状评分(IPSS)、生活质量(QOL)评分、前列腺磁共振成像(MRI)及全身骨扫描, 并记录生化复发及并发症情况。结果术后3个月, 所有患者的血清tPSA、IPSS评分和QOL评分与治疗前比较明显下降, 差异均有统计学意义(均P<0.001);术后6个月的血清tPSA水平低于术后3个月, 差异有统计学意义[(0.012±0.013)ng/mL vs.(0.096±0.174)ng/mL,P<0.05];术后1年复查前列腺体积由术前(44.85±19.66)mL缩小至术后(19.90±11.63)mL, 差异有统计学意义(P<0.05)。随访期间, 无死亡病例, 无局部进展或转移, 2例术后出现远期并发症, 1例出现生化复发。结论氩氦刀冷冻消融术治疗局限前列腺癌疗效确切, 且能缓解尿路症状, 改善生活质量, 是治疗局限性前列腺癌的可选择方式。Objective To investigate the efficacy and safety of argon-helium cryoablation for localized prostate cancer.Methods The data of 21 patients with localized prostate cancer who underwent argon-helium cryoablation in our hospital from January 2014 to December 2019 were retrospectively analyze.Serum total prostate specific antigen(tPSA),international prostate symptom score(IPSS),quality of life(QOL),MRI and whole body scan were evaluated,and biochemical recurrence and complications were recorded.Results At 3 months after surgery,the serum tPSA,IPSS and QOL scores of all patients were significantly lower than those before treatment,with statistical significance(all P<0.001).The serum tPSA level 6 months after surgery was lower than that 3 months after surgery,and the difference was statistically significant[(0.012±0.013)ng/mL vs.(0.096±0.174)ng/mL,P<0.05].Reexamination of prostate MRI one year after surgery indicated that prostate volume decreased significantly,from pre-operation(44.85±19.66)mL to post-operation(19.90±11.63)mL,with statistical significance(P<0.05).During follow-up,there were no deaths,no local progression or metastasis,two patients had long-term complications after surgery,and 1 patient had biochemical recurrence.Conclusions Cryoablation with argon-helium knife is an effective treatment for localized prostate cancer,and it can relieve urinary tract symptoms and improve quality of life.It is an optional treatment for localized prostate cancer.
