摘 要:目的对比同种异体的Bio-Gene和异种的Bio-Oss骨修复材料应用于引导组织再生术(GTR)联合植骨术在牙周骨下袋缺损的临床疗效。方法选取牙周基础治疗后仍有骨下袋缺损的牙周炎患者34例进行研究,行GTR联合植骨术的牙周再生手术,分为两组:Bio-Gene组(n=16)和Bio-Oss组(n=18)。比较两组牙周再生术前(T0)和术后12个月(T1)的探诊深度(PD)、临床附着水平(CAL)、牙龈退缩(GR)、探诊出血(BOP)、牙齿松动度(TM)、骨缺损深度(BD)、骨内缺损深度(IBD)、剩余牙槽骨高度(RBH)及骨缺损角度(α)。结果与T0相比,T1期两组的PD、CAL、BOP、TM、BD、IBD及RBH均有显著改善(P<0.05);T1期两组相比,Bio-Gene组的BD和RBH差异显著优于Bio-Oss组(P<0.05)。结论Bio-Gene和Bio-Oss骨移植材料在短时间内均能有效改善牙周骨内缺损。值得注意的是,在降低影像学骨缺损深度方面,Bio-Gene相较于Bio-Oss表现更为优越,为牙周骨内缺损的治疗及临床应用提供新的证据。Objective To compare the efficacy of allogeneic Bio-Gene and xenogenic Bio-Oss bone repair materials using in guided tissue regeneration(GTR)combined with bone for the management of intrabony defects.Methods This retrospective study included 34 patients with periodontitis who still had intrabony defects after initial periodontal therapy,and the patients were divided into the Bio-Gene group(n=16)and the Bio-Oss group(n=18)for the periodontal regeneration surgery with GTR combined with bone grafting.The periodontal probing depth(PD),clinical attachment level(CAL),gingival recession(GR),bleeding on probing(BOP),tooth mobility(TM),the distance from the cemento-enamel junction to the bottom of the bone defect(BD),intrabony defect depth(IBD),residual alveolar bone height(RBH)and bone defect angle(α)were compared between the two groups before periodontal regeneration(T0)and 12 months after periodontal regeneration(T1).Results Compared to T0,both groups showed significant improvements in PD,CAL,BOP,TM,BD,IBD,and RBH at T1(P<0.05).When compared between the two groups at T1,difference between BD and RBH in the Bio-Gene group was significantly better than that in the Bio-Oss group(P<0.05).Conclusion Both Bio-Gene and Bio-Oss can effectively improve the clinical effect of periodontal intrabony defects in a short time.It is worth noting that,in reducing BD,Bio-Gene demonstrates a superior performance compared to Bio-Oss.This provides new evidence for the treatment and clinical application of periodontal intrabony defects.
