玻璃酸钠在复合小梁切除术中的应用观察

摘　　要：目的探讨玻璃酸钠在复合小梁切除术中应用的价值及安全性。方法选取我院2017年2月~2019年6月收治的原发性闭角型青光眼患者67例(81眼),随机分为观察组33例(40眼),行常规复合小梁切除术并在术中应用玻璃酸钠,将少许玻璃酸钠分别注入巩膜瓣下及球结膜瓣下;对照组34例(41眼),单纯行复合小梁切除术。比较两组患者术后不同时间眼压、前房和滤过泡形成情况。结果两组病例术后1wk内眼压观察组高于对照组(t=7.725,P<0.05)(t=4.8148,P<0.05)(t=3.0186,P<0.05),1mon、3mon眼压控制情况相当,无统计学差异(t=1.0998,P>0.05)(t=1.2331,P>0.05);观察组在浅前房发生率上低于对照组,两组比较差异有统计学意义(χ^(2)=8.3407,P<0.05)。两组在功能性滤过泡形成方面相差不大,无统计学意义(χ^(2)=2.4893,P>0.05)。结论在复合小梁切除术中应用玻璃酸钠是安全有效的,它能减少青光眼术后早期并发症的发生,并能提高手术的成功率。Objective To investigate the value and safety of sodium hyaluronate in complex trabeculectomy.Methods A total of 67 patients(81 eyes)with primary angleclosure glaucoma admitted to our hospital from February 2017 to June 2019 were randomly divided into the observation group(33 eyes),who received conventional compound trabeculectomy and sodium hyaluronate was applied during the operation,with a little sodium hyaluronate injected into the scleral flap and bulbar conjunctiva flap respectively.In the control group,34 patients(41 eyes)were treated with compound trabeculectomy.Intraocular pressure,anterior chamber and follicular formation were compared between the two groups at different time after operation.Results Intraocular pressure observation group at 1wk after operation was higher than control group(t=7.725,P<0.05)(t=4.8148,P<0.05)(t=3.0186,P<0.05),1mon and 3mon were similar in IOP control,and there was no statistical difference(t=1.0998,P>0.05)(t=1.2331,P>0.05).The incidence of shallow anterior chamber in the observation group was lower than that in the control group,and the difference between the two groups was statistically significant(χ^(2)=8.3407,P<0.05).There was no significant difference in functional filtration bubble formation between the two groups(χ^(2)=2.4893,P>0.05).Conclusion The application of sodium hyaluronate is safe and effective in compound trabeculectomy,it can reduce the occurrence of early complications after glaucoma surgery and improve the success rate.
