眼球穿通伤并球内异物手术疗效分析

摘　　要：目的探讨眼球穿通伤并球内异物手术治疗效果。方法对2019年1月至2019年9月共9例外伤导致的眼球穿通伤并球内异物的临床资料进行回顾性分析.根据伤口位置、异物性质及滞留部位等分组,根据具体情形选择不同手术方式,通过观察术前术后视力变化情况,评估不同外伤对视力影响情况。结果一期手术均顺利取出异物,二期手术3例行人工晶体悬吊术,1例行硅油取出+人工晶体悬吊术,术后随访3~5个月,随访期末最佳矫正视力手动者2例,0.05者1例,0.1~1.2者6例,相对于术前视力,及时手术的患者,视力明显提升。术前与术后视力比较P<0.05,差异有统计学意义。结论眼球损伤病情比较复杂,需要根据具体问题具体分析才能取得较好的临床效果,预后恢复情况与受伤程度相关。Objective To investigate the effect of surgical treatment of penetrating ocular injury with intraocular foreign body.Methods Nine patients who had penetrating injuries with intrabulbar foreign bodies were analyzed from January to December 2019.According to the location of wound,the nature of foreign bodies and the location of detention,there are various surgical methods Because of the injured position,material property and location,The mechanism and the injury characteristics of the patients were evaluated by predicting the visual outcome of the victims.Results The foreign bodies were removed successfully in the first stage operation,three patients of silicone oil removal and intraocular lens suspension after primary pars plana vitrectomy.The patients were observed for three to five months after operation:Visual acuity was HM of two patients,one patient who visual acuity was less than 0.1,six patients who visual acuity was between 0.1 and 1.2.The earlier to surgery,the better get sight.Compared with the visual acuity before operation.There was significant difference in visual acuity between pre-operation and post-operation(P<0.05).Conclusion The condition of eyeball injury is more complicated,and it needs to be analyzed according to individual problems to be associated with a comparatively favorable visual prognosis.This study emphasizes that the prognosis after a penetrating injury is strongly influenced by the nature of the injury and the extent of initial damage.
