摘 要:目的观察缓哮六安煎对哮喘大鼠肺组织及Th1/Th2类细胞因子的影响,探讨其作用机制。方法取50只雄性Wistar大鼠,随机选择15只大鼠作为空白组,其余大鼠分别于第1天和第8天腹腔注射10%卵蛋白(OVA)和氢氧化铝混合溶液1 mL致敏,从第15天起予1%OVA溶液连续雾化激发7 d,每次30 min,其后选取空白组和哮喘造模大鼠各5只,通过检测气道阻力及观察肺组织病理形态评价哮喘是否造模成功。将剩余30只造模成功哮喘大鼠随机分为单纯造模组、缓哮六安煎组和地塞米松组各10只,继续给予OVA溶液雾化6周(隔日1次,溶液浓度每周递增0.5%)。于雾化前30 min,地塞米松组给予地塞米松混悬液0.0675 mg/kg灌胃,缓哮六安煎组给予缓哮六安煎溶液0.9 g/mL灌胃,空白组和单纯造模组给予蒸馏水灌胃,灌胃量均为1 mL/100 g,1次/d,连续6周。HE和Masson染色观察肺组织病理形态,ELISA法检测血清及支气管肺泡灌洗液中干扰素-γ(IFN-γ)、白细胞介素-4(IL-4)水平,计算IFN-γ/IL-4比值。结果HE和Masson染色显示单纯造模组大鼠支气管上皮细胞受损严重,基底膜明显增生肥厚,细支气管壁及周围见炎性细胞浸润,胶原蛋白沉积增多;与单纯造模组比较,地塞米松组和缓哮六安煎组大鼠病理变化均有不同程度减轻,气道周围有少量炎症细胞浸润,胶原沉积明显减少。与空白组比较,单纯造模组CVF、Wat/Pbm、Wam/Pbm和血清及支气管肺泡灌洗液中IL-4水平均明显增高(P均<0.05),血清及支气管肺泡灌洗液中IFN-γ水平和IFN-γ/IL-4比值均明显降低(P均<0.05);与单纯造模组比较,地塞米松组和缓哮六安煎组CVF、Wat/Pbm、Wam/Pbm及血清IL-4水平均明显降低(P均<0.05),缓哮六安煎组血清IFN-γ/IL-4比值及地塞米松组支气管肺泡灌洗液中IFN-γ水平均明显升高(P均<0.05);地塞米松组Wat/Pbm明显低于缓哮六安煎组(P<0.05),地塞米松组和缓哮六安煎组其余各指标比较差异均Objective It is to observe the effect of Huanxiao Liuan Decoction on lung pathology and Th1/Th2 cytokines in asthmatic rats,and to explore its mechanism.Methods Fifty male Wistar rats were selected,in which 15 ones were randomly selected as blank group,and the remaining rats were sensitized by intrabitoneally injected 1 ml of 10%OVA and Al(OH)3 mixture solution on the 1st and 8th days,respectively.From the 15th day,the rats were stimulated by continuous atomization with 1%OVA solution for 7 days,each time for 30 min.Then 5 rats in the blank control group and the asthma model group were selected to detect airway resistance and observe lung histopathology to assess whether the asthma models were successfully modeled.The remaining 30 successfully modeled asthmatic rats were randomly divided into simple modeling group,Huanxiao Liuan decoction group and dexamethasone group,with 10 rats in each group,continuously treated with OVA solution nebulization for 6 weeks(every other day,with the solution concentration increasing by 0.5%per week).30 minutes before nebulization,the dexamethasone group was given dexamethasone suspension 0.0675 mg/kg by gavage,the Huanxiao Liuan decoction group was given Huanxiao Liuan decoction solution 0.9 g/mL by gavage,and the blank group and simple model group were given distilled water by gavage,and the volume of gavage was 1 mL/100 g,1 time/d,continuously treated for 6 weeks.The pathological morphology of lung tissue was observed by HE and Masson staining,and the levels of interferon-γ(IFN-γ)and interleukin-4(IL-4)in serum and bronchoalveolar lavage flui were detected by ELISA,the ratio of IFN-γ/IL-4 was calculated.Results HE and Masson staining showed that the bronchial epithelial cells of rats in the simple model group were severely damaged,with obvious hyperplasia and hypertrophy of the basement membrane,inflammatory cell infiltration in and around the walls of the fine bronchi and increased collagen deposition;compared with the simple model group,the pathological changes in the rats
