摘  要:目的 探讨EB病毒不同抗体与EB病毒DNA联合检测鼻咽癌的诊断价值。方法 选择2018年4月—2022年6月本院收治的200例鼻咽癌患者为观察组,同期选择156名健康体检者为对照组,均实施EB病毒不同抗体、EB病毒DNA检测,以病理检查结果为金标准,汇总两组检测结果、不同分期者检测结果,统计各个指标阳性检出率及诊断效能,分析联合与单一诊断价值。结果 观察组Rta-IgG抗体、EBV-DNA与对照组比差异显著,且Ⅳ期Rta-IgG抗体、EBV-DNA高于Ⅰ期、Ⅱ期、Ⅲ期(P<0.05);观察组患者血清中三种抗体为阳性,而对照组则相反,大部分为阴性(P<0.05);以病理检查结果为金标准,VCA-IgA抗体+Rta-IgG抗体+EA-IgA抗体+EBV-DNA阳性率高于EB病毒抗体阳性率、EB病毒DNA阳性率(P<0.05);VCA-IgA抗体+Rta-IgG抗体+EA-IgA抗体+EBV-DNA检测准确度高于其他检测方案(P<0.05)。结论 在鼻咽癌诊断中应用EB病毒不同抗体及EB病毒DNA联合方案可提高诊断准确率,为临床疾病的诊断及分期提供参考,值得借鉴及实施。Objective To explore the value of different antibodies against EB virus and EB virus DNA combining detection for the diagnosis of nasopharyngeal carcinoma.Methods 200 nasopharyngeal carcinoma patients who were admitted to our hospital from April 2018 to June 2022 were enrolled in the observation group,156 healthy individuals were selected as the control group during the same period,and EB virus antibodies and EB virus DNA tests were performed among an the subjects.The pathological examination results were used as the gold standard to summarize the test results of two groups and different stages.The positive detection rate and diagnostic efficacy of each indicator were calculated,and the combined and single diagnostic values were analyzed.Results There were significant differences in Rta-IgG antibodies and EBV-DNA between the observation group and the control group,and the levels of Rta-IgG antibodies and EBV-DNA in stageⅣwere higher than those in stagesⅠ,Ⅱ,andⅢ(P<0.05).The three antibodies in the serum of the observation group patients were positive,while the control group was the opposite,with most of them being negative(P<0.05).Using pathological examination results as the gold standard,the positive rate of VCA IgA antibody+Rta IgG antibody+EA IgA antibody+EBV-DNA was higher than that of EB virus antibody and EB virus DNA(P<0.05).The accuracy of VCA-IgA antibody+Rta-IgG antibody+EA-IgA antibody+EBV-DNA detection was higher than other detection schemes(P<0.05).Conclusions The application of EB virus antibodies and EB virus DNA combination protocol in the diagnosis of nasopharyngeal carcinoma can improve diagnostic accuracy,provide reference for clinical disease diagnosis and staging,and is worth learning from and implementing.
