摘 要:目的探讨宫颈脱落细胞中序列相似性家族19成员(FAM19A4)及微小RNA(miR)124-2甲基化检测在宫颈癌筛查分流管理中的应用价值。方法选取214例宫颈病变患者作为研究对象,符合以下条件之一:人乳头瘤病毒(HPV)阴性且细胞学显示为非典型鳞状细胞(ASC-US)或低级别鳞状上皮内病变(LSIL)、HPV16或18型感染、持续高危型HPV感染。所有患者均行宫颈脱落细胞甲基化FAM19A4/miR124-2检测和病理检查,比较不同病理类型患者宫颈脱落细胞中甲基化FAM19A4/miR124-2表达情况,并绘制ROC曲线,检验甲基化FAM19A4/miR124-2对宫颈病变较小异常者高级别鳞状上皮内病变(HSIL)或以上病变的筛查效能。结果宫颈脱落细胞中甲基化FAM19A4/miR124-2阳性患者出现HSIL或以上病变的概率增加,绘制ROC曲线,以LSIL为参考,宫颈病变较小异常者宫颈脱落细胞中甲基化FAM19A4/miR124-2筛查HSIL、宫颈癌的曲线下面积为0.813(95%CI:0.713,0.913,P<0.05)。结论宫颈脱落细胞中甲基化FAM19A4/miR124-2检测在发现宫颈≥HSIL病变中具有较高临床价值,对宫颈病变较小异常患者有分流价值。Objective To explore the application value of family with sequence similarity 19 member A4(FAM19A4)and microRNA(miR)124-2 methylation detection in cervical exfoliated cells in shunt management of cervical cancer screening.Methods The study included 214 patients with cervical lesions who met one of the following criteria:human papillomavirus(HPV)-negative and cytologically characterized as atypical squamous cell(ASC-US)or low-grade squamous intraepithelial lesions(LSIL),HPV type 16 or 18 infection,and persistent high-risk HPV It's infected.FAM19A4/miR124-2 methylation in cervical exfoliated cells and pathological examination were performed in all patients.The expression of FAM19A4/miR124-2 methylation in cervical exfoliated cells of patients with different pathological types was compared,and ROC curves were drawn.To examine the efficacy of methylated FAM19A4/miR124-2 in screening for high-grade squamous intraepithelial lesions(HSIL)or more in patients with minor and abnormal cervical lesions.Results The positive rate of methylated FAM19A4/miR124-2 in cervical exfoliated cells increased with the severity of cervical lesions.ROC curve was drawn.Taking LSIL as a reference,the area under the curve of methylation FAM19A4/miR124-2 in cervical exfoliated cells for screening HSIL and cervical cancer was 0.813(95%CI:0.713,0.913,P<0.05).Conclusion The detection of methylated FAM19A4/miR124-2 in cervical exfoliated cells has high clinical value in detecting cervical lesions≥HsIL,and has shunt value for patients with small abnormal cervicallesions.
