七味白术理肠方联合脐灸治疗激素依赖溃疡性结肠炎的临床疗效及对激素撤退的影响

摘　　要：目的观察七味白术理肠方联合脐灸治疗激素依赖溃疡性结肠炎(UC)的临床疗效及对激素撤退的影响。方法将90例激素依赖UC患者按照随机数字表法分为2组,对照组45例予口服美沙拉嗪肠溶片治疗,治疗组45例予七味白术理肠方口服联合脐灸治疗。治疗3个月后,比较2组治疗前后中医证候评分、Baron内镜评分、T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))水平变化,统计2组疗效及激素撤退率。结果治疗组总有效率88.89%(40/45),对照组总有效率62.22%(28/45),治疗组临床疗效优于对照组(P<0.05)。2组治疗后中医证候各项评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05)。2组治疗后Baron内镜评分均较本组治疗前降低(P<0.05),且治疗组治疗后低于对照组(P<0.05)。2组治疗后CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均较本组治疗前升高(P<0.05),且治疗组治疗后均高于对照组(P<0.05),2组治疗后CD8^(+)均降低(P<0.05),且治疗组治疗后低于对照组(P<0.05)。治疗组激素撤退率高于对照组(P<0.05)。结论七味白术理肠方联合脐灸能有效改善激素依赖UC患者的中医证候疗效,促进肠黏膜损伤愈合及激素撤退,提高患者的免疫功能,具有良好的临床疗效。Objective To observe the clinical effect of Qiwei Baizhu Lichang Formula combined with umbilical moxibustion on steroid-dependent ulcerative colitis(UC)and its influence on hormone withdrawal.Methods Ninety patients with steroid-dependent UC were randomly assigned to the control group(n=45)and the treatment group(n=45),in which they were treated with oral mesalazine enteric-coated tablets and oral Qiwei Baizhu Lichang Formula combined with umbilical moxibustion,respectively.Treated for 3 months,the aim was to compare traditional Chinese medicine(TCM)syndrome score,Baron endoscopy scores,T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+)).The curative effect and thyroid hormone withdrawal(THW)rate were assessed.Results The overall effective rate in the treatment group was better than that in the control group(88.89%[40/45]vs 62.22%[28/45],[P<0.05],respectively).After treatment,TCM symptom scores in the both groups were significantly decreased(P<0.05),which decreased notably in the treatment group compared with the control group(P<0.05).Baron endoscopy scores in the both groups were significantly decreased(P<0.05),which were significantly lower in the treatment group than in the control group(P<0.05).After treatment,significantly increased CD3^(+),CD4^(+),CD4^(+)/CD8^(+),and decreased CD8^(+)were detected in the both groups(all P<0.05),and the treatment group was superior to the control group for CD3^(+),CD4^(+),CD4^(+)/CD8^(+),CD8^(+)(all P<0.05).The THW rate was significantly higher in the treatment group than in the control group(P<0.05).Conclusion For patients with steroid-dependent UC,Qiwei Baizhu Lichang Formula combined with umbilical moxibustion has good clinical efficacy,which can effectively improve the efficacy of TCM syndrome,promote intestinal mucosal injury healing and hormone withdrawal,and improve immune function.
