血清ESM-1、E-cad对ACI-LAA溶栓后HT的预测价值

摘　　要：目的 探讨血清内皮细胞特异性分子-1 (ESM-1)、E-钙黏连蛋白(E-cad)对大动脉粥样硬化型脑梗死(ACI-LAA)溶栓后出血转化(HT)的预测价值。方法 选取2020年1月至2022年12月新疆医科大学第一附属医院收治的110例ACI-LAA患者,根据是否发生HT分为HT组和non-HT组。对比两组基础资料及血清ESM-1、E-cad水平。采用ROC曲线分析ESM-1、E-cad预测ACI-LAA患者发生HT的价值。结果 HT组患者NIHSS评分高于non-HT组,梗死面积大于non-HT组(P <0.05),HT组患者血清ESM-1、E-cad水平也高于non-HT组(P<0.05)。Logistic回归分析显示,梗死面积大、NIHSS评分高、血清ESM-1和E-cad水平升高是ACI-LAA患者发生HT的危险因素(P<0.05)。ESM-1联合E-cad预测ACI-LAA患者发生HT的曲线下面积为0.859,预测灵敏度为84.6%,特异度为72.6%。结论 血清ESM-1和E-cad水平与ACI-LAA患者HT密切相关,可作为早期预测发生HT的参考指标。Objective To investigate the predictive value of serum endothelial cell specific molecule-1(ESM-1) and E-cadherin(E-cad) on hemorrhage transformation after thrombolysis in acute cerebral infarction(ACI)with large-artery atherosclerosis(ACI-LAA).Methods A total of 110 ACI-LAA patients admitted to the First Affiliated Hospital of Xinjiang Medical University from January 2020 to December 2022 were selected and divided into HT group and non-HT group according to whether they had HT or not.The basic data and serum ESM-1 and E-cad levels were compared between the two groups.The ROC curve was used to analyze the value of ESM-1 and E-cad in predicting HT in ACI-LAA patients.Results The NIHSS score in HT group was higher than that in non-HT group,and the infarct size was larger than that in non-HT group(P <0.05).The serum ESM-1 and E-cad levels in HT group were also higher than those in non-HT group(P <0.05).Logistic regression analysis showed that large infarct size,high NIHSS score and elevated serum ESM-1 and E-cad levels were risk factors for HT in ACI-LAA patients(P <0.05).The area under the curve of ESM-1 combined with E-cad predicted HT in ACI-LAA patients was 0.859,the predictive sensitivity was 84.6%,and the specificity was 72.6%.Conclusion The levels of serum ESM-1 and E-cad are closely related to HT in ACI-LAA patients,and can be used as reference indexes for early prediction of HT.
