摘 要:目的探讨血清S100钙结合蛋白B(S100B)、低氧诱导因子-1α(HIF-1α)水平与癫痫患者病情及认知功能的相关性。方法选取2019年1月~2022年6月苏州大学附属常熟医院收治的113例癫痫患者为癫痫组,45例健康体检者为健康对照组。检测两组研究对象血清S100B、HIF-1α水平,采用Pearson法分析癫痫患者血清S100B、HIF-1α水平与蒙特利尔认知量表(MoCA)评分的相关性。结果癫痫组患者血清S100B、HIF-1α水平显著高于健康对照组[(0.63±0.16)μg/L vs.(0.23±0.09)μg L、(0.74±0.23)ng/ml vs.(0.35±0.12)ng/ml],差异有统计学意义(t=15.949、10.551,P<0.05)。全身性发作癫痫患者血清S100B、HIF-1α水平及国立癫痫发作严重程度评估量表(NHS3)评分均显著高于局灶性发作患者[(0.68±0.17)μg/L vs.(0.60±0.15)μg L、(0.84±0.25)ng/ml vs.(0.68±0.21)ng/ml、(14.16±3.06)分vs.(9.98±2.53)分],差异有统计学意义(t=2.441、3.723、7.804,P<0.05)。与认知正常患者相比,认知障碍患者血清S100B、HIF-1α水平及NHS3评分显著升高[(0.73±0.16)μg/L vs.(0.59±0.14)μg L、(0.91±0.29)ng/ml vs.(0.67±0.18)ng/ml、(13.52±3.26)分vs.(10.72±2.98)分],MoCA评分降低[(23.35±2.06)分vs.(28.43±1.16)分],差异有统计学意义(t=5.025、5.240、4.374、16.854,P<0.05)。癫痫组血清S100B、HIF-1α水平与NHS3评分呈正相关(r=0.387、0.394,P<0.05),与MoCA评分呈负相关(r=-0.439、-0.546,P<0.05)。血清S100B、HIF-1α联合诊断癫痫患者认知功能障碍的曲线下面积分别为0.893,联合诊断特异度及AUC显著高于单独检测(P<0.05)。结论癫痫患者血清S100B、HIF-1α水平显著升高,且与患者病情严重程度及认知状况密切相关,两者可用于癫痫患者认知障碍的辅助诊断。Objective To investigate the correlation of serum S100 calcium binding protein B(S100B)and hypoxia-inducible factor-1α(HIF-1α)levels with the condition and cognitive function of patients with epilepsy.Methods 113 patients with epilepsy in our hospital from January 2019 to June 2022 were selected as the epilepsy group,and 45 healthy subjects were selected as the control group.The levels of serum S100B and HIF-1αin the two groups were detected.The correlation between serum S100B and HIF-1αlevels and Montreal cognitive scale(MoCA)scores in patients with epilepsy was analyzed by pearson method.Results The serum levels of S100B and HIF-1αin the epilepsy group were significantly higher than those in the control group[(0.63±0.16)μg/L vs.(0.23±0.09)μg L,(0.74±0.23)ng/ml vs.(0.35±0.12)ng/ml](t=15.949,10.551,P<0.05).The serum levels of S100B and HIF-1αand the score of NHS3 in patients with generalized seizures were significantly higher than those in patients with partial seizures[(0.68±0.17)μg/L vs.(0.60±0.15)μg L,(0.84±0.25)ng/ml vs.(0.68±0.21)ng/ml,(14.16±3.06)points vs.(9.98±2.53)points](t=2.441,3.723,7.804,P<0.05).Compared with the patients with normal cognition,the patients with cognitive impairment had significantly higher serum levels of S100B and HIF-1αand NHS3 score[(0.73±0.16)μg/L vs.(0.59±0.14)μg L,(0.91±0.29)ng/ml vs.(0.67±0.18)ng/ml,(13.52±3.26)points vs.(10.72±2.98)points],and had significantly lower MoCA score[(23.35±2.06)points vs.(28.43±1.16)points](t=5.025,5.240,4.374,16.854,P<0.05).The serum levels of S100B and HIF-1αwere positively correlated with NHS3 score and negatively correlated with MoCA score in the epilepsy group(P<0.05).The area under the curve of serum S100B and HIF-1αcombined diagnosis of cognitive dysfunction in epilepsy patients was 0.893,and the specificity and AUC of combined diagnosis were significantly higher than those of single detection(P<0.05).Conclusion The serum levels of S100B and HIF-1αare significantly increased in patients with epilepsy,and ar
