摘　　要：胰腺癌是一种隐匿起病、迅速进展的高度恶化的消化系统肿瘤,由于其高侵袭性、细胞外基质的物理屏障作用和解剖位置位于腹膜后且被空腔脏器遮挡等原因,胰腺实体瘤的早期诊断率极低;又由于胰腺有极丰富的淋巴引流,并与胆道、十二指肠等器官及腹膜后相沟通,所以在胰腺癌早期便常有广泛的淋巴结转移,影响手术切除的效果及预后。自1987年开始,作为研究热点的外泌体其细胞间通讯方式的功能持续被发现,尤其是其内的miRNA家族可对靶细胞的功能进行精准定向调控,在胰腺癌病程和进展中以及化疗耐药中扮演重要角色,对胰腺癌的临床研究、抑制化疗耐药以及寻找新型治疗及检查靶点方面有着重要意义。Pancreatic cancer is a malignant tumor of the digestive system with hidden onset,rapid progress and high deterioration.Due to its high aggressiveness,the physical barrier effect of extracellular matrix,and the anatomical location located retroperitoneally and obscured by hollow organs,the early diagnosis rate of pancreatic solid tumors is extremely low.In addition,because the pancreas has abundant lymphatic drainage and communicates with the biliary tract,duodenum and retroperitoneum,there is often extensive lymph node metastasis in the early stage of pancreatic cancer,which affects the effects and prognosis of surgical resection.Since 1987,exosomes,as a hot research topic,have been continuously discovered for their functions as intercellular communication methods.In particular,the miRNAs in them can accurately and directionally regulate the functions of target cells,and play an important roles in the course and progress of pancreatic cancer and chemotherapy resistance,which is of great significance for clinical research on pancreatic cancer,inhibition of chemotherapy resistance and finding new treatments and targets.
