摘 要:目的分析宫颈癌患者高危型人乳头瘤病毒(HPV)基因型与阴道微生态的相关性。方法选取2020年1月至2022年12月九江市第三人民医院的110例宫颈癌与健康检查者作为研究对象,根据是否患宫颈癌分为宫颈癌组(80例)和对照组(30例)。两组均进行HPV基因型检测。观察宫颈癌组患者的高危HPV感染情况、不同临床分期、组织类型、不同分化程度;比较宫颈癌组与对照组的阴道微生态情况。比较高危HPV基因型患者的阴道微生态情况。结果80例宫颈癌患者中,高危HPV感染率为88.75%,宫颈癌组患者高危HPV基因型感染前三位为HPV16(58.75%)、HPV18(11.25%)、HPV58(7.70%)。宫颈癌HPV感染患者的pH值≥4.5、乳酸杆菌异常、细菌性阴道炎、衣原体、解脲脲原体、滴虫比例高于对照组,差异有统计学意义(P<0.016)。HPV16、HPV18、HPV58、混合感染患者的阴道pH值≥4.5、乳酸杆菌异常、细菌性阴道炎、衣原体、解脲脲原体、滴虫比例比较,差异无统计学意义(P>0.05)。HPV16、HPV18、HPV58均与pH≥4.5、乳酸杆菌异常、细菌性阴道炎、衣原体、解脲脲原体、滴虫存在相关性(P<0.05)。结论宫颈癌高危HPV基因型感染以HPV16、HPV18、HPV58及混合感染多见,高危HPV感染可导致患者阴道微生态紊乱。Objective To analyze the correlation between high-risk human papillomavirus(HPV)genotypes and vaginal microbiota in cervical cancer patients.Methods A total of 110 cervical cancer patients and health checkups from the Jiujiang Third People's Hospital from January 2020 to December 2022 were selected as the research subjects.They were divided into a cervical cancer group of 80 patients and a control group of 30 patients based on whether they had cervical cancer.Both groups underwent HPV genotype testing.The high-risk HPV infection status,different clinical stages,tissue types,and differentiation degrees of cervical cancer patients were observed.The vaginal microbiota between the cervical cancer group and the control group was compared.The vaginal microbiota of high-risk HPV genotype patients was compared.Results Among 80 cervical cancer patients,the high-risk HPV infection rate was 88.75%.The top three high-risk HPV genotype infections in the cervical cancer group were HPV16(58.75%),HPV18(11.25%),and HPV58(7.70%).The ratio of pH≥4.5,lactobacillus abnormality,bacterial Vaginitis,chlamydia,ureaplasma urealyticum and trichomonas in cervical cancer patients with HPV infection were higher than that in the control group,the differences were statistically significant(P<0.017).There were no statistically significant differences in the proportion of vaginal pH≥4.5,abnormal lactobacilli,bacterial vaginitis,chlamydia,ureaplasma urealyticum,and trichomonas among patients with HPV16,HPV18,HPV58,and mixed infection(P>0.05).HPV16,HPV18,and HPV58 were correlated with pH≥4.5,abnormal lactobacilli,bacterial vaginitis,chlamydia,ureaplasma urealyticum,and trichomonas(P<0.05).Conclusion HPV16,HPV18,HPV58,and mixed infections are more common among high-risk HPV genotypes in cervical cancer,and high-risk HPV infection can lead to vaginal microbiota disorders in patients.
