摘　　要：目的探讨枣仁安神胶囊联合齐拉西酮治疗首发急性期精神分裂症患者的疗效及对事件相关电位(ERP)N400的影响。方法以2022年1月至2023年1月浙江省温州市第七人民医院收治的82例首发急性期精神分裂症患者作为研究对象。按治疗方法将其分为对照组(n=41,给予齐拉西酮治疗)和观察组(n=41,给予齐拉西酮联合枣仁安神胶囊治疗)。比较2组的临床疗效、阴性和阳性症状量表(PANSS)评分、主客观睡眠质量、N400水平。结果观察组临床总有效率95%较对照组80%高(χ^(2)=4.100,P=0.043)。观察组治疗后PANSS评分较对照组低(t=5.462、P=0.000)。与对照组比较,观察组治疗后总睡眠时间延长更明显,睡眠效率升高更明显,睡眠潜伏期及觉醒时间缩短更明显,匹兹堡睡眠质量指数(PSQI)评分降低明显(t=6.637、4.161,11.850、9.610、4.514、P均<0.05)。与对照组比较,观察组治疗后匹配与非匹配N400潜伏期均缩短更明显,N400波幅均回升更明显(t=2.418、P=0.018,t=2.908、P=0.005,t=2.300、P=0.029,t=2.286、P=0.025)。2组总不良反应发生率比较差异无统计学意义(χ^(2)=0.082、t=0.775)。结论将枣仁安神胶囊与齐拉西酮联合用于首发急性期精神分裂症患者中,可改善其临床精神症状、睡眠质量及认知功能,疗效确切,安全性较高。Objective To explore the efficacy of Zaoren Anshen Capsule combined with ziprasidone in the treatment of first-episode acute schizophrenia patients and its impact on event related potential(ERP)N400.Methods A total of 82 first-episode acute schizophrenia patients admitted to Wenzhou Seventh People′s Hospital from January 2022 to January 2023 were selected as the subjects of this study.They were divided into a control group(n=41,treated with ziprasidone)and an observation group(n=41,treated with ziprasidone combined with Zaoren Anshen Capsules)depending on the method of treatment.The clinical efficacy,negative and positive symptom scale(PANSS)scores,subjective and objective sleep quality,and N400 level of the two groups were compared.Results The total clinical effective rate of the observation group was 95%,which was higher than that of the control group at 80%(χ^(2)=4.100,P=0.043).The PANSS score in the observation group after treatment was lower than that in the control group(t=5.462,P<0.01).Compared with the control group,the observation group showed a more significant increase in total sleep time,sleep efficiency,sleep latency and wakefulness time,and a more significant decrease in Pittsburgh Sleep Quality Index(PSQI)score after treatment(t=6.637,4.161,11.850,9.610,4.514,all P<0.01).Compared with the control group,the observation group showed a more significant reduction in both matched and unmatched N400 latency and a more significant increase in N400 amplitude after treatment(t=2.418,2.908,2.300,2.286,all P<0.01).There was no significant difference in the overall incidence of adverse reactions between the two groups(χ^(2)=0.082,t=0.775).Conclusion The combination of Zaoren Anshen Capsule and ziprasidone can improve clinical psychiatric symptoms,sleep quality,and cognitive function in first-episode acute schizophrenia patients,with definite therapeutic effect and high safety.
