摘　　要：目的:分析腹腔镜根治术对早期胆囊癌患者的治疗效果。方法:将88例早期胆囊癌患者随机分为对照组(n=44)和观察组(n=44)。对照组行开腹根治术,观察组行腹腔镜根治术,对比2组术前及术后1个月的围手术期指标、免疫应激反应、生活质量、并发症发生情况。结果:观察组术中出血量、手术时间、胃肠功能恢复时间和住院时间等各项围手术期相关指标优于对照组,术后的肾上腺素、去甲肾上腺素、皮质醇、白介素-4水平低于对照组,干扰素-γ水平与GQOLI-74内各维度评分高于对照组,并发症发生率低于对照组(均P<0.05)。结论:腹腔镜根治术治疗早期胆囊癌患者效果显著,手术创伤性小,对机体免疫应激反应更轻,可有效提高患者生活质量。Objective:To analyze the therapeutic effect of laparoscopic radical surgery in patients with early gallbladder cancer.Methods:Eighty-eight patients with early gallbladder cancer were randomly divided into a control group(n=44)and an observation group(n=44).The control group underwent open radical surgery and the observation group underwent laparoscopic radical surgery.The perioperative indexes,immune stress response,quality of life,and the incidence of complications were compared between the two groups before and 1 month after surgery.Results:The perioperative indicators such as intraoperative bleeding,the time for operation and gastrointestinal function recovery and hospitalization were better in the observation group than the control group.The postoperative levels of epinephrine,norepinephrine,cortisol and interleukin-4 and the incidence of complications were lower,while the levels of interferon-gamma and the scores of dimensions in the GQOLI-74 were higher in the observation group than the control group(all P<0.05).Conclusion:Laparoscopic radical surgery for the treatment of patients with early gallbladder cancer is effective with less invasive injury and lighter immune stress response to the organism and can effectively improve the quality of life of patients.
