热牙胶根管充填和冷侧方加压治疗牙髓病和根尖周病的临床疗效

摘　　要：目的探讨热牙胶根管充填和冷侧方加压治疗牙髓病和根尖周病的临床疗效。方法选取100例于2022年7月至2023年2月在兰溪市中医院口腔科采用C型根管治疗的牙髓病和根尖周病患者作为本次前瞻性研究的研究对象。将入选病例随机分为对照组和试验组,每组各50例。对照组患者采用冷牙胶侧方加压充填根管治疗法,试验组患者采用热牙胶根管充填治疗法。术后采用锥形束CT观察充填效果,比较两组患者完成根管充填的时间;采用数字分级评分法(numerical rating scale,NRS)比较两组患者牙周根尖疼痛情况。观察两组患者治疗后不良反应发生情况。结果试验组根管填充适充率(94.00%)明显高于对照组(76.00%)(P<0.05);试验组综合疗效总有效率(98.00%)明显高于对照组(86.00%)(P<0.05);试验组根管充填时间明显低于对照组(P<0.05);治疗后两组患者牙周根尖疼痛NRS评分均低于治疗前(P<0.05),治疗后试验组患者牙周根尖疼痛NRS评分明显低于对照组,差异有统计学意义(P<0.05)。两组患者不良反应差异比较无统计学意义(P>0.05)。结论相较于冷侧方加压充填根管治疗,热牙胶根管充填治疗牙髓病和根尖周病患者适充率和综合疗效更好,可有效减轻患者的疼痛、缩短根管充填时间且不良反应较少,具有较好的临床适用性。Objective To investigate the clinical efficacy of hot gutta-percha root canal filling and cold lateral compression in the treatment of endodontic and periapical diseases.Methods A total of 100 patients with pulp disease and periapical disease treated with C-type root canal therapy in Department of Stomatology of Lanxi Hospital of Traditional Chinese Medicine were selected as the research subjects for this prospective study from July 2022 to February 2023.The selected cases were randomly divided into control group and trial group,each group of 50 cases.The control group patients were treated with cold tooth glue side pressure filling root canal,and the test group patients were treated with hot tooth glue filling root canal.After the operation,the filling effect was observed by conical beam CT,and the time of root canal filling was compared between the two groups.A numerical rating scale(NRS)was used to compare periodontal root tip pain in two groups of patients.To observe the occurrence of adverse reactions after treatment in two groups of patients.Results The filling rate of root canal in test group(94.00%)was significantly higher than that of control group(76.00%)(P<0.05).The total effective rate(98.00%)of the experimental group was significantly higher than that of the control group(86.00%)(P<0.05).The root canal filling time in the test group was significantly lower than that in the control group(P<0.05).The NRS scores of periodontal root tip pain in both groups were lower than those before treatment(P<0.05).The NRS score of periodontal root tip pain in the experimental group was significantly lower than that of the control group(P<0.05).There was no statistical difference between the two groups(P>0.05).Conclusion Compared with the cold side pressure filling root canal treatment,the hot gutta-percha root canal filling has better filling rate and comprehensive curative effect in patients with pulp disease and periapical disease,which can effectively reduce the pain of patients and shorten the root canal filling
