上颌窦内提升术联合短种植体治疗上颌后牙缺失伴轻度骨量不足的疗效观察

摘　　要：目的:观察上颌窦内提升术联合短种植体治疗上颌后牙缺失伴轻度骨量不足的疗效。方法:选择2019年1月至2020年6月本院收治的176例上颌后牙缺失伴轻度骨量不足患者作为研究对象。根据患者选择的治疗方式将患者分成对照组(91例)和试验组(85例)。对照组使用上颌窦内提升术加常规种植体治疗,试验组采用上颌窦内提升术联合短种植体进行治疗。全部患者均完成18 m随访。分析对比修复术后1 w、1 m、6 m、1 y、1.5 y种植体颈部边缘骨吸收量、种植体存留率、不良反应发生率及患者治疗满意度。结果:两组颈部边缘骨吸收量相比差异无统计学意义(P>0.05)。两组患者的种植体存留率对比无显著差异(P>0.05)。试验组不良反应发生率低于对照组(P<0.05)。试验组治疗总满意度高于对照组(P<0.05)。结论:使用上颌窦内提升术联合短种植体对上颌后牙缺失伴轻度骨量不足患者治疗可提升患者安全性,减少不良反应,提高治疗满意度。Objective:To observe the efficacy of maxillary sinus lift combined with short implants in the treatment of maxillary posterior tooth loss with mild bone mass deficiency.Methods:From January 2019 to June 2020,176 patients with maxillary posterior tooth loss and mild bone deficiency were selected as the study objects.The patients were divided into control group(91 cases)and experimental group(85 cases)according to the treatment mode chosen by the patients.The control group was treated with maxillary sinus lift combined with conventional implant,and the experimental group was treated with maxillary sinus lift combined with short implant.All patients completed 18 months of follow-up.Bone uptake,implant retention rate,incidence of adverse reactions and patient satisfaction were analyzed and compared 1 week,1 month,6 months,1 year and 1.5 years after repair.Results:There was no significant difference in the bone absorption of neck margin between the two groups(P>0.05).There was no significant difference in implant retention between the two groups(P>0.05).The incidence of adverse reactions in experimental group was lower than that in control group(P<0.05).The total satisfaction of experimental group was higher than that of control group(P<0.05).Conclusion:The use of internal maxillary sinus lifting combined with short implants in the treatment of maxillary posterior tooth loss with mild bone deficiency can improve patient safety,reduce adverse reactions,and improve treatment satisfaction.
