摘 要:目的 分析超声影像组学特征对肝泡型包虫病(HAE)患者阿苯达唑脂质体治疗的预测价值.方法 收集2020年9月至2023年9月我院收治的257例HAE患者的临床资料,将患者随机分为训练集(207例)和验证集(50例).训练集患者根据阿苯达唑脂质体治疗后的疗效分为有效组(122例)和无效组(85例).比较两组患者的临床资料和超声影像组学特征.通过多因素分析影响患者阿苯达唑脂质体治疗效果的独立危险因素.构建列线图模型,并通过Bootstrap重采样对列线图进行内部验证.结果 257例肝泡型包虫病HAE患者在阿苯达唑脂质体治疗后有效153例,无效104例.多因素分析结果显示黄疸、肺肾脑转移、病灶大小、病灶数目、钙化、液化坏死、血流信号均为肝泡型包虫病患者阿苯达唑脂质体治疗效果的影响因素(P<0.05).列线图模型的ROC曲线下面积为0.797,敏感度为70.09%,特异度为70.52%,准确度高达71.03%.校准曲线分析,列线图模型的拟合效果显著.模型净收益远高于影像学和临床特征.结论 超声影像组学特征对肝泡型包虫病患者阿苯达唑脂质体治疗具有一定预测价值,可作为预测患者治疗效果的临床依据.Objective The predictive value of analyzing ultrasound imaging omics characteristics for albendazole liposome therapy in patients with hepatic alveolar echinococcosis.Methods Collect clinical data of 257 patients with hepatic alveolar echino-coccosis admitted to our hospital from September 2020 to September 2023,and randomly divide them into a training set(207 cases)and a validation set(50 cases).The training set patients were divided into an effective group(122 cases)and an ineffective group(85 cases)based on the efficacy of albendazole liposome treatment.Compare the clinical data and ultrasound imaging omics character-istics of two groups of patients.Independent risk factors affecting the efficacy of albendazole liposome therapy in patients were analyzed through multiple factor analysis.Build a column chart model and perform internal validation on the column chart through Bootstrap resa-mpling.Results Among 257 patients with hepatic alveolar echinococcosis,153 were effective and 104 were ineffective after treatment with albendazole liposomes.Univariate analysis showed significant differences in treatment efficacy in terms of jaundice,lung kidney brain metastasis,lesion size,number of lesions,solid partial echo,calcification,liquefaction necrosis,and blood flow signals(P<0.05).Multifactor analysis results show that the size of lesions,number of lesions,calcification,liquefaction necrosis,and blood flow signal are all influencing factors for the effiicacy of albendazole liposome therapy in patients with hepatic alveolar echinococcosis(P<0.05).The area under the ROC curve of the line graph model is 0.817,and the C-index is 0.810,the calibration curve has a good fit,and the clinical decision curve threshold probability has a high net benefit value in the range of 0.04~0.95.This indicates that the line graph model has high discrimination,good accuracy,good effectiveness,and is safe and reliable.Conclusion The ultrasound imaging omics characteristics have certain predictive value for albendazole liposome therapy in p
