摘  要:目的研究^(13)C-尿素呼气试验采用端口“一测一消毒”对幽门螺杆菌感染检测中潜在污染因素的质控影响。方法入选2022年4月至7月行^(13)C-尿素呼气试验受检者3055例,采用常规直接吹气和端口消毒改良检测方法,分析DOB值特征、总阳性率和不同测量通道使用率及阳性率差异,分析采用端口“一测一消毒”质控措施消除潜在污染对提高测量结果准确性的价值。结果常规方法和改良方法测量的DOB值及阳性率差异没有统计学意义,但是改良方法测量的DOB值数据一致性更好,离散趋势小,需要再次复查的DOB临界值数值减少。不同通道检测的阳性率在两种方法中差异均存在统计学意义(P<0.0045)。结论^(13)C-尿素呼气试验临床实际应用中存在潜在交叉感染风险(用嘴直接吹气),但对检测结果阳性率未见明显影响,对数据稳定性有一定影响,在工作条件和受检者配合度允许的条件下,建议采取端口“一测一消毒”质控方法避免潜在交叉感染风险,提高检测数据稳定性,减轻患者医疗负担。Objective To evaluate the effect of“one test,one disinfection”quality control on the detection results of ^(13)C-urea breath test(^(13)C-UBT)through comparative analysis.Methods From April to July 2022,a total of 3055 clients were measured the infection of H.pylori by ^(13)C-UBT and the characteristics of DOB value were analyzed.Furthermore,the utilization rates of different measurement channels and the difference of positive rates between each channel also compared under the scenarios of standard measurement and improved measurement of“one test,one sterilization”.Results There was no significant difference between the DOB values measured by the standard and improved measurement,but the DOB values measured from improved method showed less dispersion of data and less patients who need re-examination.With the standard measurement,there was a significant difference in the utilization rate of each channel as well as a significant difference in positive rate of H.pylori infection(P<0.0045),while with the improved measurement of“one test,one sterilization”,the utilization rate of each channel was relatively balanced,and although there was still a significant difference in positive rate(P<0.0045)but the smaller difference was observed in improved method.Conclusion Disinfection of the collection port is a simple and easy quality control that could improve accuracy of the ^(13)C-UBT for H.pylori infection with a large number of daily tests in huge hospital.
