磁共振断层血管成像对原发性三叉神经痛临床诊断的价值

摘　　要：目的探讨原发性三叉神经痛患者行磁共振断层血管成像诊断的临床价值。方法本次纳入2017年1月至2022年6月本院就诊的280例原发性三叉神经痛患者进入研究,术前患者给予磁共振断层血管成像检查,将手术结果作为金标准。观察磁共振断层血管成像的诊断效能。结果2位医生对患者患侧、健侧的神经-血管关联性一致程度较高(Kappa患侧=0.85,Kappa健侧=0.79),患者患侧神经血管阳性率95.4%,66例健侧神经假阳性率为23.6%,接受手术治疗患者120例手术中发现有118例存在神经血管接触压迫。结论三叉神经痛患者应用磁共振断层血管成像扫描检查能够帮助医生了解其神经血管状态,为判断病因以及后续制定手术治疗方案提供可靠数据参考,更好地保障患者的治疗效果及健康。Objective To explore the clinical value of magnetic resonance tomography and vascular imaging in patients with primary trigeminal neuralgia.Methods The neurovascular association between two doctors on the affected side and the unaffected side was high(Kappaaffected side=0.85,Kappahealthy side=0.79).The postive rate of the affected nerve vessel was 95.4%,and the positive rate of the healthy nerve was 23.6%.There were 120 patients treated with surgery,and 118 patients were found to have neurovascular contact/compression during surgery.Conclusion The use of magnetic resonance tomography and vascular imaging scan for patients with trigeminal neuralgia can help doctors to understand their neurovascular status,provide reliable data reference for judging the cause and making the subsequent surgical treatment plan,and better ensure the treatment effect and health of patients.
