血清NLR、CEA、AFP、CA125对胃癌腹膜转移的评估价值

摘　　要：目的 探讨血清中性粒细胞/淋巴细胞比值(NLR)、血清肿瘤标志物癌胚抗原(CEA)、甲胎蛋白(AFP)、糖类抗原125(CA125)对胃癌腹膜转移的评估价值。方法 收集胃癌患者85例,按是否发生腹膜转移分为未转移组(n=50)与转移组(n=35),随访1年,观察腹膜转移组生存预后情况。检测血清NLR、CEA、AFP、CA125水平,分析其与胃癌腹膜转移及生存预后的关系。结果 转移组血清NLR、CEA、AFP、CA125水平明显高于未转移组(t=5.371、1.001、3.613、3.619,P均<0.05),胃癌腹膜转移患者血清NLR、CEA、AFP、CA125之间互相呈正相关(P<0.05)。转移组患者术后随访1年,生存30例,生存患者血清NLR、CEA、AFP、CA125水平明显低于死亡患者。受试者工作特征曲线(ROC)分析显示:NLR、CEA、AFP、CA125中CA125预测胃癌腹膜转移患者不良预后效能最高,其约登指数最大时对应cut-off为47.07 U/ml,曲线下面积(AUC)为0.920,预测不良预后的敏感度、特异性分别为80.00%、93.33%。结论 胃癌腹膜转移的患者血清NLR、CEA、AFP、CA125均明显异常上调,且与患者生存预后有关,通过监测上述因子可评估胃癌腹膜转移及预后。Objective To investigate and analyze the evaluated value of serum neutrophil-to-lymphocyte ratio(NLR)and serum tumor markers of carcinoembryonic antigen(CEA),alpha-fetoprotein(AFP)and carbohydrate antigen 125(CA125)on gastric cancer with peritoneal metastasis,and to provide evidence for evaluating peritoneal metastasis in patients with gastric cancer.Methods 85 patients with gastric cancer were collected and divided into non-metastasis group(n=50)and metastasis group(n=35)according to whether peritoneal metastasis occurred.Serum levels of NLR,CEA,AFP and CA125 were detected,and the survival prognosis was compared between the 2 groups,and the relationship between the above factors and peritoneal metastasis and survival prognosis of gastric cancer was explored.Results Serum levels of NLR,CEA,AFP and CA125 in metastasis group were significantly higher than those in non-metastasis group(t=5.371,1.001,3.613,3.619,P<0.05),and serum NLR,CEA,AFP and CA125 levels in patients with gastric cancer and peritoneal metastasis were positively correlated with each other(P<0.05).Receiver operating characteristic curve(ROC)analysis revealed that among NLR,CEA,AFP and CA125,CA125 had the highest efficiency on predicting poor prognosis in patients with gastric cancer complicated with peritoneal metastasis.When the Youden index of CA125 was maximum,the corresponding cut-off value,area under the curve(AUC),sensitivity and specificity for predicting poor prognosis were 47.07 U/ml,0.920,80.00%and 93.33%respectively.Conclusion Serum NLR,CEA,AFP and CA125 are significantly abnormally up-regulated in patients with gastric cancer complicated with peritoneal metastasis,and they are related to survival prognosis of patients.It is possible to evaluate peritoneal metastasis and prognosis of gastric cancer by monitoring the above factors.
