摘 要:目的 观察在巨块型肝癌的介入治疗-载药微球栓塞(DEB-TACE)中应用CalliSpheres载药微球治疗肝细胞癌的疗效与安全性。方法 选取78例肝细胞癌患者,按照治疗方式将其分为研究组(n=38)和常规组(n=40),其中研究组采用DEB-TACE中应用CalliSpheres载药微球治疗,常规组采用传统肝动脉化疗栓塞术(TACE)进行治疗,比较2组患者治疗3个月后的临床疗效,比较2组患者治疗前和治疗后生化[白蛋白(ALB)、血清总胆红素(TBIL)、谷丙转氨酶(ALT)]水平变化,比较2组患者治疗前和治疗后肿瘤标志物[血清甲胎(AFP)、异常凝血酶原(APT)]水平变化,比较治疗后2组患者不良反应发生情况。结果 研究组的临床疗效显著优于常规组(P<0.05)。治疗前,2组患者的ALB、TBIL和ALT均无显著差异(P>0.05);治疗后,2组患者的ALB均较治疗前显著上升,且研究组显著高于常规组(P<0.05),2组患者的TBIL和ALT均较治疗前显著下降,且研究组显著低于常规组(P<0.05)。研究组的行为毒性、神经系统、心血管系统的分数均显著小于常规组(P<0.05),2组患者的实验室检查、植物神经系统、其他的分数均无显著差异(P>0.05)。结论 DEB-TACE中应用CalliSpheres载药微球可有效提高肝细胞癌介入治疗的临床疗效,在改善肝功能的同时,能有效抑制肿瘤标志物表达,且安全性良好,具有一定临床应用价值。Objective To observe the efficacy and safety of CalliSpheres drug-eluting beads during drug-eluting bead-based transcatheter arterial chemoembolization(DEB-TACE)for massive hepatocellular carcinoma(HCC).Methods 78 patients with HCC were selected and divided into the study group(n=38)and the conventional group(n=40)according to treatment methods.The study group was given CalliSpheres drug-eluting beads during DEB-TACE while the conventional group was treated with traditional transcatheter arterial chemoembolization(TACE).The clinical efficacy after 3 months of treatment,changes in biochemical indicators[albumin(ALB),serum total bilirubin(TBIL),alanine aminotransferase(ALT)]before and after treatment,changes in tumor markers[serum alpha-fetoprotein(AFP),abnormal prothrombin(APT)]before and after treatment and occurrence of side-effect after treatment were compared between the 2 groups.Results The clinical efficacy in the study group was significantly better than that in the conventional group(P<0.05).Before treatment,there were no significant differences in the levels of ALB,TBIL and ALT between the 2 groups(P>0.05).After treatment,the ALB level in the 2 groups of patients was significantly increased compared with that before treatment,and the level in study group was significantly higher than that in conventional group(P<0.05).The levels of TBIL and ALT in the 2 groups were significantly decreased compared with those before treatment,and the levels in study group were significantly lower compared to conventional group(P<0.05).The scores of behavioral toxicity,nervous system and cardiovascular system in the study group were significantly lower than those in the conventional group(P<0.05),but there were no significant differences in the scores of laboratory examination,vegetative nervous system and others between the 2 groups(P>0.05).Conclusion The application of CalliSpheres drug-eluting beads during DEB-TACE can effectively enhance the clinical efficacy of interventional therapy for hepatocellular carcinoma,and ca
