神经浸润与胃癌临床病理因素及其预后的关系

摘　　要：目的 分析神经浸润(PNI)与胃癌(GC)其他临床病理因素及其预后的关系。方法 回顾性分析274例GC患者的临床资料,按照有无PNI将患者分为PNI阳性组(n=65)和PNI阴性组(n=209),分析PNI与GC患者临床病理因素的关系和预后情况。结果 PNI阳性与GC患者分化程度、TNM分期、T分期、N分期、脉管癌栓存在明显相关性(P<0.05);logistic回归分析显示,分化程度为低分化、TNM分期为Ⅲ期、T分期为T4期、N分期为N3期、有脉管癌栓是影响GC患者PNI阳性的危险因素(P<0.05);GC患者预后与肿瘤直径、TNM分期、脉管癌栓、PNI明显相关(P<0.05);logistic回归分析显示,肿瘤直径>5 cm、TNM分期为Ⅲ期、有脉管癌栓、PNI阳性是影响GC患者预后的危险因素(P<0.05)。结论 PNI与GC患者的分化程度、TNM分期等临床病理因素存在相关性,且存在PNI的GC患者往往预后不良,5年生存率较低,临床可将PNI作为诊断和评估GC预后的一项重要参考指标,为临床治疗提供新思路。Objective To analyze the relationship between nerve infiltration(PNI)and other clinicoppathological prognostic expression of gastric cancer(GC).Methods Clinical data of 274 GC patients were retrospectively analyzed and divided into PNI positive(n=65)and PNI negative(n=209)in the presence of PNI,and the relationship between PNI and clinicopathological factors in GC patients were analyzed.Results Positive PNI was significantly correlated with the differentiation degree,TNM stage,T stage,N stage,and vascular cancer plugs in GC patients(P<0.05);The logistic regression analysis revealed that low differentiation degree,TNM stage was stageⅢ,T stage was T4 stage,N stage was N3 stage,and vascular cancer thrombolysis were the risk factors affecting PNI positivity in GC patients(P<0.05);The prognosis of GC patients was significantly associated with tumor diameter,TNM stage,vascular and cancer embolus,and PNI(P<0.05);The logistic regression analysis revealed that tumor diameter>5 cm,TNM stage,vascular cancer thrombus,and positive PNI were the risk factors affecting the prognosis of GC patients(P<0.05).Conclusion There is a correlation between PNI and clinicopathological factors such as differentiation degree and TNM stage of GC patients,and GC patients with PNI often have poor prognosis and low 5-year survival rate.PNI can be used as an important reference index to diagnose and evaluate GC prognosis,and provide new ideas for clinical treatment.
