摘 要:目的探讨磁共振弥散加权成像(DWI)联合动态增强磁共振(DCE-MRI)对卵巢肿瘤良恶性鉴别诊断的价值。方法收集2022年2月至2023年2月经术后病理证实为卵巢恶性肿瘤的68例患者为试验组,同期术后病理证实为卵巢良性肿瘤的68例患者为对照组。两组患者术前均行DWI和DCE-MRI检查,获得以下参数:DWI相关(囊性和实性部分的ADC值)、DCE-MRI相关容量转移常数(K^(trans))、血管外细胞外体积分数(V_(e))、速率常数(K_(ep))及时间-信号曲线(TIC)特征,进行计数和分析,评价DWI和DCE-MRI单独以及联合对卵巢肿瘤的诊断效能(敏感度、特异度和准确度)。结果DWI和DCE-MRI联合诊断的敏感度(97.06%)和准确度(97.06%)高于DWI(82.35%,88.97%)、DCE-MRI(80.88%,89.71%)(P<0.05);而DWI(95.59%)和DCE-MRI(98.53%)诊断特异度差异无统计学意义(P>0.05)。结论DWI联合DCE-MRI可提高卵巢肿瘤良恶性鉴别诊断的诊断价值。Objective To investigate the value of diffusion-weighted imaging(DWI)combined with dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)in the differential diagnosis of benign and malignant ovarian tumors.Methods 68 patients with ovarian malignant tumors in hospital from February 2022 to February 2023 were selected as the experimental group,and another 68 patients with benign ovarian tumors in the same period were the control group.Both groups of patients should complete the examinations of DWI and DCE-MRI.The following parameters were obtained:DWI-related apparent diffusion coefficient(ADC)values for the cystic and solid components,DCE-MRI-related transfer constant(K^(trans)),extravascular extracellular volume fraction(V_(e)),rate constant(K_(ep))and time-signal curve(TIC)characteristics.The parameters were analyzed to evaluate the diagnostic performance(sensitivity,specificity,and accuracy)of DWI,DCE-MRI,and their combination for differentiating diagnosis of benign and malignant ovarian tumors.Results The sensitivity(97.06%)and accuracy(97.06%)of the combined diagnosis were higher than those of DWI(82.35%,88.97%)and DCE-MRI(80.88%,89.71%)(P<0.05);There was no difference in the diagnostic specificity between DWI(95.59%)and DCE-MRI(98.53%)(P>0.05).Conclusion The combination of DWI and DCE-MRI can improve the diagnostic value in distinguishing between the benign and malignant ovarian tumors.
