基于“形神一体观”探讨中晚期肝癌辨治

摘　　要：“形神一体观”来源于我国朴素的古代哲学思想,形神两者互根互生,相互依赖,相互影响。形体有恙,劳心伤神;心神不宁,形体失养。“形神一体观”在肝癌的发生发展中有着重要意义。古代医家认为,肝癌病证属本虚邪实。肝癌多由人体正气虚损,阴阳失和,情志不舒日久,致使肝失条达,肝失疏泄,肝郁气滞,津液不布,聚湿为痰,气滞痰凝,瘀阻经络,肝脾损伤,气机瘀滞,久郁化火,内停瘀血,热毒湿蕴,渐积所成。在临床上,早期肝癌,多采用手术方法切除肿块,患者患病时间短,病情较轻,术后能较快回到正常生活,心理负担较轻。而中晚期肝癌,术后多需辅助放化疗、靶向治疗、免疫治疗等,治疗周期长,压力陡增,极易造成情志失调,诱发一系列临床症状。由此可见,长期情志失调,情绪抑郁,不仅增强恶性肿瘤易感性,促进恶性肿瘤发生与发展,还可促使肿瘤血管生成,加速肿瘤转移。中晚期肝癌从调摄“形神”论治,在“形”健脾,在“神”解郁的主导思想下,通过固本健脾、解郁散结等药物,联合情绪疏导,调整机体和情志平和,尽可能延长生存期,提高生活质量,达到事半功倍的效果。The concept of the unity between the body and the Shen(神)comes from the simple ancient philosophical thought of China,and the two are rooted in each other,interdependent and mutually influential.The body is sick,and the heart is sad.Restlessness and physical loss.The concept of the unity between the body and the Shen is of great significance in the occurrenceT and development of liver cancer.Ancient physicians believed that the syndrome of liver cancer was a false reality.Liver cancer is mostly caused by the deficiency of righteous Qi(气)in the human body,the disharmony of Yin(阴)and Yang(阳),and the long-term emotional discomfort,resulting liver loss its smoothly moving state,liver stagnafion,liver depressed Qi stagnation,deficiency of body fluid,Gathering dampness turn into phlegm,Qi stagnafion phlegm condense,blockage of meridians,Liver and spleen injury,Qi movement stagnation,liver depression turn into fire,intravenous stop blood stasis,heat poison and dampness,and gradual accumulation.Clinically,in early stage liver cancer,surgical methods are mostly used to remove the mass,and the patient has a short illness time,mild disease,and can return to normal life quickly after surgery,and the psychological burden is lighter.However,in the middle and advanced stages of liver cancer,adjuvant chemoradiotherapy,targeted therapy,immunotherapy,etc.are often required after surgery,and the treatment cycle is long and the pressure increases sharply,which can easily cause emotional imbalance and induce a series of clinical symptoms.It can be seen that long-term emotional imbalance and depression not only enhance the susceptibility to malignant tumors,promote the occurrence and development of malignant tumors,but also promote tumor angiogenesis and accelerate tumor metastasis.Under the leading idea of the unity between the body and the Shen and strengthening the spleen and relieving depression by Shen,combined with emotional counseling,the body and emotional peace can be adjusted,the survival period can be prolonged as m
