脑血流动力学指标和颈动脉内膜中层厚度与抑郁症关系研究

摘　　要：目的:分析脑血流动力学指标和颈动脉内膜中层厚度(CIMT)与抑郁症的关系。方法:纳入抑郁症患者114例为抑郁症组,纳入体检健康者90例为正常组。比较两组一般资料、脑血流动力学指标及CIMT。比较两组汉密尔顿抑郁量表24项(HAMD-24)评分,分析抑郁症患者脑血流动力学指标、CIMT与HAMD-24评分的相关性。结果:两组性别、家庭关系和家庭收入比较差异有统计学意义(均P<0.05)。抑郁症组双侧大脑前动脉(ACA)、大脑中动脉(MCA)、颈内动脉末端(TICA)及基底动脉(BA)的收缩峰流速(Vs)、舒张末期流速(Vd)、平均流速(Vm)低于正常组,CIMT高于正常组(均P<0.05)。两组左侧ACA、左侧TICA、双侧MCA、BA的脉动指数(PI)和阻力指数(RI)比较差异无统计学意义(均P>0.05)。抑郁症组HAMD-24评分为高于正常组(P<0.05)。双侧ACA、MCA、TICA及BA的Vs、Vd、Vm与HAMD-24评分呈负相关(均P<0.05),PI、RI与HAMD-24评分无相关性(均P>0.05)。CIMT与HAMD-24评分呈正相关(P<0.05)。结论:抑郁症患者脑血流动力学指标及CIMT存在异常,且与患者抑郁程度存在密切的联系。Objective:To analyze the association of cerebral hemodynamic indexes and carotid intima-media thickness(CIMT)with depression.Methods:A total of 114 patients with depression were enrolled as the depression group,and 90 healthy people were enrolled as the normal group.The general data,cerebral hemodynamic indexes and CIMT were compared between the two groups.The HAMD-24 scores were compared between the two groups,and the relationship between cerebral hemodynamic indexes,CIMT and HAMD-24 scores in patients with depression was analyzed.Results:There were significant differences in gender,family relationship and family income between the two groups(all P<0.05).The Vs,Vd and Vm of bilateral anterior cerebral artery(ACA),middle cerebral artery(MCA),terminal internal carotid artery(TICA)and basilar artery(BA)in depression group were lower than those in normal group,and CIMT was higher than that in normal group(all P<0.05).There were no significant differences in Pulsatility Index(PI)and Resistance Index(RI)of left ACA,left TICA,bilateral MCA and BA between the two groups(all P>0.05).The HAMD-24 score of the depression group was higher than that of the normal group(P<0.05).The Vs,Vd and Vm of bilateral ACA,MCA,TICA and BA were negatively correlated with HAMD-24 scores(all P<0.05),while PI and RI were not correlated with HAMD-24 scores(all P>0.05).CIMT was positively correlated with HAMD-24 score(P<0.05).Conclusion:The cerebral hemodynamic indexes and CIMT are abnormal in patients with depression,which are closely related to the degree of depression.
