摘 要:目的通过测量不同矢状骨性错畸形患者锥形束CT(CBCT)的牙弓及基骨宽度数据并作对比分析,从而揭示不同矢状骨性错畸形患者的牙弓及基骨宽度间的差异和相关性。方法选取2020年1月至2023年2月于德阳市人民医院口腔科/口腔正畸专科就诊的165例骨性Ⅰ类、150例骨性Ⅱ类及102例骨性Ⅲ类错畸形患者的CBCT,测量其牙弓及基骨宽度数据;以单因素方差分析及LSD法分析不同矢状骨性错畸形患者的牙弓及基骨宽度的差异,以Pearson检验分析牙弓与基骨宽度之间的相关性。结果与骨性Ⅰ类患者相比,骨性Ⅱ类患者的下颌牙弓宽度(-1.75±0.38)mm及基骨宽度(-3.23±0.43)mm均更小,差异有统计学意义(P<0.001);而骨性Ⅲ类患者的下颌牙弓宽度(1.57±0.42)mm及基骨宽度(3.85±0.48)mm均更大,差异有统计学意义(P<0.001);3种不同矢状骨性错畸形患者的上下颌牙弓与基骨宽度之间均存在一定相关性(P<0.001)。结论骨性Ⅱ类患者的下颌牙弓及基骨较骨性Ⅰ类患者更窄,而骨性Ⅲ类患者的下颌牙弓及基骨较骨性Ⅰ类患者更宽大,牙弓与基骨宽度相匹配。Objective To measure and compare the width of maxillary and mandibular dental arch and basal bone by measuring cone-beam computed tomography(CBCT)data of different sagittal skeletal malocclusion,so as to find the differences and correlation of basal bone and dental arch width in different sagittal skeletal malocclusion.Methods CBCT scans of 165 patients with skeletal ClassⅠmalocclusion,150 patients with skeletal ClassⅡmalocclusion and 102 patients with skeletal ClassⅢmalocclusion in the Department of Stomatology/Orthodontics of Deyang People's Hospital from January 2020 to February 2023 were selected for the measurement of dental arch and basal bone width.One-Way ANOVA analysis and Least Significant Difference(LSD)method were used to analyze the differences of dental arch and basal bone width in different sagittal skeletal malocclusion.Pearson test was used to analyze the correlation between the dental arch and basal bone width.Results Compared with skeletal ClassⅠpatients,the mandibular dental arch width(-1.75±0.38)mm and basal bone width(-3.23±0.43)mm of skeletal ClassⅡpatients were significantly smaller(P<0.001),but the mandibular dental arch width(1.57±0.42)mm and basal bone width(3.85±0.48)mm of skeletal ClassⅢpatients were significantly bigger(P<0.001).There was a certain correlation between dental arch and basal bone width in three different sagittal skeletal malocclusion(P<0.001).Conclusion Compared with patients with skeletal ClassⅠmalocclusion,patients with skeletal ClassⅡmalocclusion have narrower mandibular dental arch and basal bone,but patients with skeletal ClassⅢmalocclusion have wider mandibular dental arch and basal bone.The dental arch width matches the width of the basal bone.
