摘　　要：目的探究直肠癌根治术中倒U型切开邓氏筋膜保护血管神经束的效果。方法选取2020年1月—2021年12月瓦房店市中心医院、大连医科大学附属新华医院、上海交通大学附属瑞金医院行直肠癌根治术的340例患者为研究对象,根据术式不同分为对照组(n=170)和研究组(n=170),对照组采用常规直肠癌根治术,研究组采用在直肠癌根治术中实施邓氏筋膜倒U型切开保护血管神经束技术,比较两组手术时间、术后泌尿功能、性功能的差异以及术后并发症发生情况。结果两组手术时间、术后并发症发生率对比差异无统计学意义(P>0.05)。研究组术后泌尿功能障碍、勃起功能障碍、射精功能障碍发生率明显低于对照组(P<0.05)。结论直肠癌根治术中实施倒U型切开邓氏筋膜保护血管神经束技术能够有效降低患者术后泌尿功能障碍、勃起功能障碍及射精功能障碍,且不会增加手术并发症,安全性良好,值得推广应用。Objective To investigate effect of Deng's fascial inverted u-shaped incision for vascular and nerve bundle protection during radical resection of rectal cancer.Methods The paper collected 340 patients with rectal cancer radical surgery at Wafangdian Central Hospital,Dalian Medical University Affiliated Xinhua Hospital,and Shanghai Jiao Tong University Affiliated Ruijin Hospital from January 2020 to December 2021 as study subjects,and divided them into control group(n=170)and study group(n=170)according to different surgical methods.Control group was treated with routine rectal cancer radical surgery,and study group with Deng's fascial inverted u-shaped incision for vascular and nerve bundle protection during rectal cancer radical surgery.Difference of surgical time,postoperative urinary and sexual function,as well as incidence of postoperative complications was was compared between two groups.Results There was no statistically significant difference in surgical time and postoperative complication incidence between two groups(P>0.05).Incidence of postoperative urinary dysfunction,erectile dysfunction,and ejaculatory dysfunction in study group was significantly lower than control group(P<0.05).Conclusion Deng's fascial inverted u-shaped incision for vascular and nerve bundle protection during radical resection of rectal cancer,can reduce postoperative urinary,erectile and ejaculatory dysfunction of patients effectively,with good safety and without increasing surgical complications,which is worthy of promotion and application.
